摘要:目前,大剂量糖皮质激素的使用是造成我国非创伤性股骨头坏死的主要原因,严重影响患者的健康和生活质量。根据临床治疗与现代实验研究,中医药治疗激素性股骨头坏死疗效确切,备受学界关注。OPG/RANK/RANKL信号通路的发现成为激素性股骨头坏死防治的突破口,激素性股骨头坏死"脾虚生痰-由痰致瘀-因瘀致痹"的病机与该信号通路密切相关。文章基于从"瘀"论治思想中医渊源及激素性股骨头坏死病理特点,并结合现代医学脂质代谢、炎症因子、细胞凋亡及OPG/RANK/RANKL信号通路对骨代谢的调控机制阐述其相关性,探究从"瘀"论治思想在激素性股骨头坏死防治中的合理性,旨在为激素性股骨头坏死的精准诊断和靶向、个体化治疗提供理论依据。
Currently,The use of high-dose glucocorticoids is the main cause of non-traumatic femoral head necrosis in China,seriously affect the patients’ health and quality of life. According to clinical treatment and modern experimental research,traditional Chinese medicine has a definite curative effect on the treatment of hormonal femoral head necrosis,which has attracted the attention of academia. The discovery of OPG/RANK/RANKL signaling pathway has become a breakthrough for the prevention and treatment of hormonal femoral head necrosis,the pathogenesis of hormonal femoral head necrosis,"spleen deficiency produces phlegm-stasis from phlegm-paralysis due to stasis"is closely related to this signaling pathway. This article is based on the origin of traditional Chinese medicine from the perspective of"stasis"and the pathological characteristics of hormonal femoral head necrosis,and combined with modern medicine lipid metabolism,inflammatory factors,apoptosis and OPG/RANK/RANKL signaling pathways to regulate bone metabolism to explain its correlation,to explore the rationality of treating hormonal femoral head necrosis from the perspective of"stasis",it aims to provide a theoretical basis for the accurate diagnosis and targeted,individualized treatment of hormonal femoral head necrosis.
