摘要:变应性鼻炎(allergic rhinitis,AR),作为一种普遍的耳鼻喉科疾病,在全球范围的患者数量庞大,对患者日常生活影响较大。尽管现代医学在AR的治疗上取得了一定的进展,但仍较难根治该疾病,且现有治疗方案伴随着较大的不良反应和不尽理想的长期疗效。相比之下,中医药在AR的治疗中展现出显著的疗效和独特的治疗优势。在中医学中,AR被归类为“鼻鼽”的范畴,其病理机制通常被认为是本虚标实、虚实夹杂。黄芪作为一种重要的补气药材,在AR的治疗中得到了广泛应用。黄芪多糖(astragalus polysaccharide,APS)作为黄芪的主要活性成分,通过多靶点调控AR的免疫失衡与炎症级联反应,展现出显著的临床潜力。文章系统综述APS在AR中的分子机制,重点聚焦其通过下调胸腺基质淋巴生成素(TSLP)表达、调节T-bet/GATA-3与Foxp3/RORγt转录因子平衡、抑制NF-κB信号通路及NLRP3炎症小体活化等途径,纠正Th1/Th2及Th17/Treg细胞失衡,从而缓解鼻黏膜炎症。结合近年研究,文章进一步探讨APS在纳米递送技术提升生物利用度、中西医联合治疗中的转化价值,为AR的精准干预提供理论依据。
Allergic rhinitis(AR),as a common otorhinolaryngology disease,has a large number of patients in the world and our country,which has a great impact on the daily life of patients.Although modern medicine has made some progress in the treatment of AR,it is still difficult to cure the disease completely,and the existing therapeutic schemes are accompanied by great side effects and unsatisfactory long-term efficacy.In contrast,traditional Chinese medicine(TCM)has shown significant efficacy and unique advantages in the treatment of AR.In traditional Chinese medicine,AR is classified as“Biqiu”,characterized by deficiency-excess complexity and its pathological mechanism is generally considered to be a mixture of deficiency and superficiality.As an important Chinese herbal medicine for invigorating Qi,Huangqi(Astragali Radix)has been widely used in the treatment of AR.As the main active ingredient of Huangqi(Astragali Radix),Astragalus polysaccharide(APS)has shown significant clinical potential through multi-target regulation of immune imbalance and inflammatory cascade in AR.In this paper,the molecular mechanisms of APS in AR are systematically reviewed,this paper focuses on its role in regulating the expression of TSLP,regulating the balance of T-bet/GATA-3 and Foxp3/RORγt transcription factors,inhibiting the NF-κb signaling pathway and NLRP3 inflammasome activation,to correct the imbalance of Th1/Th2 and Th17/Treg cells,thereby alleviating nasal mucosal inflammation.Combined with recent studies,this paper further discusses the transformation value of APS in nano-delivery technology to improve bioavailability and combined treatment of traditional Chinese and Western medicine,and provides a theoretical basis for the precise intervention of AR.
