摘要：当前生活方式和环境的改变导致少弱精子症成为男性不育最常见的原因之一。中医认为,该病的本虚主要体现在肾精亏虚、阴阳失调,标实则多为气滞、湿热、血瘀。通过基础研究发现,利用中药活性成分(罗布麻宁、淫羊藿苷、非瑟酮、金丝桃苷、芸香苷、菟丝子黄酮、虾青素、薯蓣皂苷元、银松素、五味子甲素等中药活性成分)、中药及中成药(调控HIF-1α/EGFR/MAPK1、P53/Bax/Caspase-3、Keap1/Nrf2、JAK2/STAT3、IL-17等信号通路)通过抗氧化、抗凋亡、调节激素水平等途径改善精子质量。临床研究中采用滋阴补肾方、生精胶囊、乌鸡白凤丸、龟龄集等中药复方在改善临床症状方面显示出独特优势,但也存在临床研究缺乏多中心、大样本和机制探索不足等缺陷。
Current lifestyle and environmental changes have caused oligoasthenospermia(OAS)to be one of the most common causes of male infertility.According to traditional Chinese medicine(TCM),the root deficiency of this disease mainly manifests as deficiency of kidney essence and imbalance of yin and yang,while the superficial excess is primarily characterized by qi stagnation,damp-heat,and blood stasis.Basic research has found that active ingredients of Chinese medicinals(CMs)(such as apocynin,icariin,fisetin,hyperoside,rutin,cuscuta flavonoids,astaxanthin,diosgenin,piceatannol,schisandrin A,etc.)and CMs and patent medicines(regulating HIF-1α/EGFR/MAPK1,p53/Bax/caspase-3,Keap1/Nrf2,JAK2/STAT3,IL-17 and other signaling pathways)can ameliorate sperm quality through antioxidant,anti-apoptotic,and hormone-regulating effects.In clinical studies,Chinese herbal medicine compound prescriptions such as Ziyin Bushen Decoction(滋阴补肾方,ZYBSD),Shengjing Capsule(生精胶囊,SJC),Wuji Baifeng Pill(乌鸡白凤丸,WJBFW),and Guilingji(龟龄集,GLJ)have shown unique advantages in alleviating clinical symptoms,but there are also deficiencies such as the lack of multi-center,large-sample studies and insufficient exploration of mechanisms.
