摘要：目的 对国内外DRG研究现况和趋势进行文献分析和总结,为我国DRG实践与研究提供参考。方法 收集中国知网和Web of Science数据库中1980年—2021年的DRG研究文献,通过描述性统计方法、VOSviewer与CiteSpace可视化分析相结合的方法,对国内外DRG文献历年发文量、发文期刊、发文作者、发文机构、研究热点和研究趋势等进行分析研究。结果 在研究热点方面,国内关注DRG对住院费用的影响、临床路径与DRG的协同应用、病案首页填写质量等几个方面;国外侧重DRG与病例死亡风险、流行病学、成本管理等方面的相关研究,病例分组与疾病编码的研究,以及DRG对住院费用(费用补偿)和医疗服务质量的影响等几个方面。在研究趋势方面,国内主要关注是按病种付费的改革成效及其与DRG比较研究、医保制度对医保基金和基金支付能力的影响、公立医院病案首页质量研究等三个方面;国外侧重基于DRG的全髋关节置换外科手术成效及医生绩效相关研究、基于DRG数据记录的费用与出院率比较研究、DRG临床编码等三个方面。结论 国内DRG研究机构间合作较少,研究方法同质化严重,创新性不足,应加强机构间的科研合作,增强DRG研究的实践实用性,提升DRG研究成果价值。
Objective To analyze and summarize the research status and trend of literature on DRG at home and abroad,so as to provide reference for DRG practice and research in China.Methods The DRG research literature from 1980 to 2021 in CNKI and the Web of Science database were collected.By means of descriptive statistics,VOSviewer and CiteSpace visual analysis,analysis was made of the number of DRG literature,publication journals,authors,publication institutions,research hotspots and research trends at home and abroad over the years.Results In terms of research hotspots,domestic researches focused on the impact of DRG on hospitalization expenses,the collaborative application of clinical pathway and DRG,and the filling quality of the first page of medical records,while foreign researches focused on the correlation of DRG with case death risk,epidemiology,cost management,as well as case grouping and disease coding,and the impact of DRG on hospitalization expenses(expense compensation) and medical service quality.In terms of research trends,domestic researches mainly focused on the reform effectiveness of payment by disease and its comparison with DRG,the impact of medical insurance system on medical insurance fund and fund payment capacity,and the quality of the first page of medical records in public hospitals,while foreign researches focused on the effectiveness of total hip arthroplasty,physician performance based on DRG,the comparative study of cost and discharge rate based on DRG data records,and DRG clinical coding.Conclusions Domestic DRG research institutions are less likely to cooperative,with serious homogenization of research methods and insufficient innovation.Inter-institutional research cooperation should be strengthened to enhance the practical utility of DRG research and improve the value of DRG research results.
