摘要：目的探究乳腺癌保乳术后美学效果客观评价工具BCCT.core(breast cancer conservative treatment.cosmetic results,BCCT.core)与日本乳腺癌学会(Japanese breast cancer society,JBCS)主观评价量表评分之间的一致性,为BCCT.core在临床应用提供可靠的循证依据。方法自2021年1月至2024年12月,甘肃中医药大学第三附属医院纳入107例接受乳腺癌保乳术的患者,其中传统保乳手术46例(43%)、整形保乳手术34例(32%)、腔镜保乳手术27例(25%)。通过BCCT.core软件对患者术后胸部正面照片进行自动化美学分析,同时3名乳腺外科医师使用JBCS量表进行评分。采用组内相关系数(ICC)评估医师间评分的一致性;Spearman等级相关系数对BCCT.core软件评分与医师评分进行相关性分析。结果医师间评分呈现中等一致性(ICC相关系数为0.63);BCCT.core软件评分与3名医师评分均值表现出高度相关性(Spearman's rho=0.87,P<0.001)。与医师使用JBCS量表评价相比,BCCT.core软件操作简便,高效,评价标准化以及结果可重复等优势。因此,在临床应用中,BCCT.core软件能够节省时间及人力物力成本。结论BCCT.core软件的评价结果与医师主观评分具有高度相关性和可比性,其客观性及可靠性使其可作为乳腺癌保乳术后美学效果评价的辅助工具。
Objective To validate the agreement between BCCT.core(breast cancer conservative treatment.cosmetic results),an objective assessment tool,and the Japanese Breast Cancer Society(JBCS)subjective scale for evaluating cosmetic outcomes after breast-conserving surgery,thereby providing reliable evidence for the clinical application of BCCT.core.Methods From January 2021 to December 2024,this study included 107 patients who underwent BCS for breast cancer,consisting of 46 cases of conventional BCS(43%),34 cases of oncoplastic BCS(32%),and 27 cases of endoscopic BCS(25%).Postoperative frontal breast photographs of the patients were subjected to automated aesthetic analysis using the BCCT.core software.Simultaneously,three breast surgeons scored the cosmetic results using the JBCS scale.Intraclass correlation coefficient(ICC)was used to assess the inter-rater consistency among the surgeons'scores;Spearman's rank correlation coefficient was used to analyze the correlation between the BCCT.core software score and the surgeons'scores.Results Inter-rater reliability among the surgeons demonstrated moderate agreement(ICC=0.63).The scores from BCCT.core showed a high correlation with the mean ratings of the three surgeons(Spearman's rho=0.87,P<0.001).Compared to the evaluation using the JBCS scale by physicians,the BCCT.core software offers advantages such as user-friendly operation,high efficiency,standardized assessment,and reproducible results.Consequently,its clinical application can save considerable time,manpower,and material resources.Conclusion The evaluation results of the BCCT.core software are highly correlated and comparable with subjective surgeon ratings.Its objectivity and reliability support its use as an auxiliary tool for assessing cosmetic outcomes after breast-conserving surgery.
