摘要：目的:探讨针药复合麻醉对老年椎间孔镜下髓核摘除术患者术中镇静、镇痛效果的影响。方法:选择甘肃中医药大学附属医院择期椎间孔镜下髓核摘除术老年患者60例,随机分为局麻复合针刺麻醉组、局麻复合右美托咪定组各30例。观察并比较两组术前(T0)、导针穿刺皮肤时(T1)、纤维环扩孔开窗时(T2)、髓核摘除时(T3)、射频消融治疗时(T4)各时间节点血流动力学变化,同时评价两组术中视觉模拟评分(Visual Analogue Scale,VAS)及镇静评分(Ramsay)。结果:两组T1~T4时间节点无创平均动脉压比较差异无统计学意义(P>0.05),心率比较差异具有统计学意义(P<0.05)。两组T1~T4时间节点VAS比较,差异有统计学意义(P<0.05),Ramsay比较于T4时,差异有统计学意义(P<0.05)。结论:针药复合麻醉应用于老年椎间孔镜下髓核摘除术中,能够维持术中血流动力学的稳定,且具有良好的镇痛及部分镇静作用。
Objective:To investigate the influence of TEAS on hemodynamic changes and sedative and analgesic effects during percutaneous transforaminal endoscopic discectomy(PTED).Methods:Sixty gerontal patients,scheduled to undergo PTED in our hospital,randomly devided to local anesthesia combined with transcutaneous electrical acupoint stimulation(TEAS)group(group A)and local anesthesia combined with dexmedetomidine group(group B).Hemodynamic changes at each time node before surgery(T0),guide needle puncture the skin(T1),fibrous rings reams(T2),removal of nucleus pulposus(T3),radiofrequency ablation(T4)were compared between the two groups,and intraoperative visual analogue scale(VAS)and ramsay were simultaneously evaluated.Results There was no significant difference in mean arterial pressure comparison in T1-T4 time node between the two groups(P>0.05),while there were significant differences in heart rate(P<0.05).There was significant difference in VAS comparison in T1-T4 time node between the two groups(P<0.05),and significant difference in Ramsay comparison in T4 time node between the two groups(P<0.05).Conclusion:Local anesthesia combined with TEAS in percutaneous transforaminal endoscopic discectomy in gerontal patients,can maintain the stability of intraoperative hemodynamics,and has good analgesic and partial sedative effects.
