摘要：类风湿关节炎(rheumatoid arthritis,RA)作为一种迁延不愈的自身免疫性疾病,其核心病理特征为关节炎症的持续发作、滑膜的异常增生,以及软骨与骨质的渐进性破坏。既往研究表明,RA发病与线粒体调控异常相关,线粒体作为细胞的“能量工厂”,在细胞存活、凋亡、代谢等多个生理过程中发挥关键作用。而线粒体质量控制(mitochondrial quality control,MQC)是维持线粒体内环境稳态的重要机制。该机制的存在,有助于确保线粒体的数量、形态和功能处于最佳状态,从而满足细胞的能量需求。MQC与RA发病之间的联系,体现在线粒体生物合成、线粒体动力学、线粒体氧化应激、线粒体自噬功能障碍,以及钙离子稳态失衡等多个层面。近年来,中医药以其独特的理论体系和多靶点、多途径的治疗优势,在调节MQC以干预RA方面展现出令人瞩目的潜力。本研究旨在系统梳理中医药靶向MQC防治RA的最新进展,期望能为RA的诊断与治疗开辟新的视角。
Rheumatoid arthritis(RA),as a chronic autoimmune disease,is characterized by the persistent inflammation of joints,abnormal proliferation of the synovium,and progressive destruction of cartilage and bone.Previous studies have shown that the onset of RA is associated with mitochondrial regulatory abnormalities.As the powerhouses of cells,mitochondria play a crucial role in various physiological processes such as cell survival,apoptosis,and metabolism.Mitochondrial quality control(MQC)is an essential mechanism for maintaining the homeostasis of the internal environment of mitochondria.This mechanism ensures that the quantity,morphology,and function of mitochondria are optimal,thereby meeting the energy demands of cells.The connection between MQC and the onset of RA manifests at multiple levels,including mitochondrial biosynthesis,mitochondrial dynamics,mitochondrial oxidative stress,dysfunction of mitochondrial autophagy,and calcium ion homeostasis imbalance.In recent years,traditional Chinese medicine has demonstrated remarkable potential in regulating MQC to intervene in RA due to its unique theoretical system and multi-target,multi-pathway therapeutic advantages.This study aims to systematically review the latest progress in using traditional Chinese medicine targeting MQC for the prevention and treatment of RA,hoping to open up new perspectives for the diagnosis and treatment of RA.
