摘要:目的基于磷脂酰肌醇3-激酶(PI3K)/蛋白激酶B(AKT)信号通路探讨护卵汤加减方干预多囊卵巢综合征(PCOS)的作用机制。方法将62只SD大鼠采用随机数字表法分为空白组、模型组、西药组、中药组4组,其中空白组8只,其余3组各18只。采用来曲唑联合1%羧甲基纤维素持续灌胃法建立PCOS大鼠模型。造模成功后,空白组、模型组灌胃生理盐水,西药组灌胃达英-35溶液,中药组灌胃护卵汤加减方浓缩液,连续灌胃21d。比较各组大鼠空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)和血清激素[促卵泡激素(FSH)、雌二醇(E_(2))、黄体生成素(LH)、抗米勒管激素(AMH)、睾酮(T)、胰岛素(INS)]水平,以及卵巢组织PI3K、AKT蛋白表达水平,通过苏木精-伊红染色观察卵巢、子宫、肝脏组织病理学改变。结果与空白组比较,模型组大鼠FBG、TG、TC、LDL-C及血清LH、AMH、T、INS水平明显升高,HDL-C和血清FSH、E_(2)水平,以及卵巢组织PI3K、AKT蛋白相对表达量明显降低,差异均有统计学意义(P<0.05或P<0.01);与模型组比较,西药组与中药组大鼠FBG、TG、TC、LDL-C及血清LH、AMH、T、INS水平明显降低,HDL-C和血清FSH、E_(2)水平,以及卵巢组织PI3K、AKT蛋白相对表达量明显升高,差异均有统计学意义(P<0.05或P<0.01)。中药组及西药组可不同程度改善PCOS模型大鼠的卵巢、子宫、肝脏组织病理学改变,有效改善卵巢形状,使颗粒细胞层变得丰满且有序分布,促进卵母细胞成熟。结论护卵汤加减方可能通过激活PI3K/AKT信号通路而抑制PCOS模型大鼠卵巢颗粒细胞凋亡,从而改善大鼠排卵功能。
Objective To investigate the mechanism of Modified Huluan Tang(护卵汤加减方)in intervening with polycystic ovary syndrome(PCOS)based on the phosphoinositide 3-kinase(PI3K)/protein kinase B(AKT)signaling pathway.Methods Sixty-two SD rats were randomly divided into four groups using a random number table:a blank group(n=8),a model group(n=18),a western medicine group(n=18),and a traditional Chinese medicine(TCM)group(n=18).The PCOS rat model was established by continuous gavage of letrozole combined with 1%carboxymethyl cellulose.After successful modeling,the blank and model groups received 0.9%normal saline via gavage,the western medicine group received Diane-35 solution,and the TCM group received concentrated Modified Huluan Tang,all for 21 consecutive days.Fasting blood glucose(FBG),triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high-density lipoprotein cholesterol(HDL-C),serum hormone[follicle-stimulating hormone(FSH),estradiol(E_(2)),luteinizing hormone(LH),anti-Müllerian hormone(AMH),testosterone(T),insulin(INS)]levels,and PI3K and AKT protein expression levels in ovarian tissue were compared.Pathological changes in ovarian,uterine,and liver tissues were observed via hematoxylin-eosin(HE)staining.Results Compared with the blank group,the model group showed significantly increased FBG,TG,TC,LDL-C,and serum LH,AMH,T,and INS levels,while HDL-C,serum FSH,E_(2)levels,and PI3K and AKT protein relative expression level in ovarian tissue were significantly decreased(P<0.05 or P<0.01).Compared with the model group,the western medicine and TCM groups exhibited significantly reduced FBG,TG,TC,LDL-C,and serum LH,AMH,T,and INS levels,while HDL-C,serum FSH,E_(2)levels,and PI3K and AKT protein relative expression level were significantly increased(P<0.05 or P<0.01).The western medicine and TCM groups ameliorated pathological changes in ovarian,uterine,and liver tissues to varying degrees,effectively improving ovarian morphology,enriching the granulosa cell layer with orderly distribution,and promoting oocyte maturation.Conclusion Modified Huluan Tang may inhibit apoptosis of ovarian granulosa cells in PCOS model rats by activating the PI3K/AKT signaling pathway,thereby improving ovulatory function.
