摘要:目的评价戊二醛、过氧化氢、环氧乙烷对腹腔镜的消毒效果,为腹腔镜消毒提供科学依据。方法以Peritoneoscope、Celioscopes、腹腔镜、戊二醛等中、英文关键词检索PubMed、Web of science、Embase(ovid)、中国生物医学文献数据库(CBM)、中国期刊网全文数据库(CNKI)等数据库,搜集与戊二醛、过氧化氢、环氧乙烷对腹腔镜消毒灭菌相关的随机对照试验(RCT)文献,进行系统评价及meta分析。结果初检文献325篇,最终纳入10篇,累计样本含量9 030例。分析结果显示:过氧化氢组消毒效果优于环氧乙烷组,过氧化氢组消毒效果优于戊二醛组,差异均有统计学意义[危险比(RR)=0.20,95%CI(0.05,0.89),P=0.03];戊二醛浸泡30 min时,环氧乙烷消毒效果优于戊二醛,差异有统计学意义[RR=0.33,95%CI(0.21,0.52),P=0.05];戊二醛浸泡600 min时,环氧乙烷组消毒效果和戊二醛组比较,差异无统计学意义[RR=1.50,95%CI(0.32,7.14),P=0.61]。结论相较于戊二醛和环氧乙烷,过氧化氢低温等离子灭菌法安全可靠,具有灭菌周期短、环保、安全性高等优点,而戊二醛和环氧乙烷在消毒效果上无明显差异。
Objective To evaluate the efficacies of glutaraldehyde, hydrogen peroxide and ethylene oxide on laparoscopic disinfection to provide a scientific basis for laparoscopic disinfection. Methods The databases of PubMed, Web science, Embase (ovid), CBM and CNKI were retrieved by Chinese and English key words "peritoneoscope", "celioscopes" and "glutaraldehyde".The randomized controlled trials (RCT) literatures on glutaraldehyde, hydrogen peroxide and ethylene oxide for disinfecting laparoscope were collected and performed the systematical assessment. Results A total of 325 articles were preliminarily screened out, and 10 articles were finally included, its accumulated sample contents were 9 030 cases.The analytic results showed that the disinfection effect in the hydrogen peroxide group ,was superior to that in the ethylene oxide group, and the disinfection effect in the hydrogen peroxide group was superior to that in the glutaraldehyde group, the difference was statistically significant[RR = 0.20,95%CI(0.05,0.89), P=0.03];when immersing laparoscope by glutaraldehyde for 30 rain, the disinfection effect of ethylene oxide was superior to that of glutaraldehyde with statistical difference[RR=0.33,95%CI (0.21, 0.52), P=0.05];when immersing laparoscope by glutaraldehyde tbr 600 rain, the disinfection effect had no statistical difference between the ethylene oxide group and glutaraldehyde group[RR=l.50,95%CI (0.32,7.14), P=0.61]. Conclusion Compared with glutaraldehyde and ethylene oxide, hydrogen peroxide low temperature plasma sterilization method is safe and reliable, and has the advantages of short cycle, environmental protection and high safety, whereas ethylene oxide and glutaraldehyde have no obvious difference in disinfection effect.
