摘要:目的观察穴位注射葛根素对盐酸布比卡因所致大鼠心脏毒性的保护作用。方法将40只SD雄性大鼠随机分为空白组(I组)、模型组(Ⅱ组)、葛根素注射液预防组(Ⅲ组)、葛根素注射液治疗组(Ⅳ组),每组10只。I组大鼠于股静脉泵入生理盐水2 mg/(kg·min),其余各组大鼠按相同方法泵入盐酸布比卡因注射液制作模型。I组大鼠于双侧内关穴注射生理盐水各0.1 m L,Ⅱ组和Ⅳ组出现心律失常时分别注射等量生理盐水和葛根素注射液,Ⅲ组在泵入盐酸布比卡因前30 min注射等量葛根素注射液。记录给局麻药后大鼠出现心律失常、心脏骤停的时间及相应的局麻药剂量,取心肌组织检测Na^+-K^+-ATP酶和乳酸脱氢酶(LDH)的活性。结果与Ⅱ组比较,Ⅲ组和Ⅳ组大鼠出现心律失常、心脏骤停时间明显延迟,对应的盐酸布比卡因累积剂量增大,大鼠心肌组织中Na^+-K^+-ATP酶活性明显升高,LDH活性显著降低,差异有统计意义(P<0.05或P<0.01);但Ⅲ组在延迟心律失常、心脏骤停时间,升高Na^+-K^+-ATP酶活性,降低LDH活性方面更显著,差异均有统计意义(P<0.05)。结论穴位注射葛根素能够减轻盐酸布比卡因的心脏毒性,其机制可能与增加心肌Na^+-K^+-ATP酶活性、降低心肌LDH活性有关。
Objective To observe the protective effect of puerarin acupoint injection on myocardial toxicity induced by bupivacaine hydrochloride in rats. Methods 40 SD adult male rats were randomly divided into four groups: blank group( group Ⅰ),model group( group Ⅱ),puerarin prevention group( group Ⅲ),puerarin treatment group( group Ⅳ). Group Ⅰ was pumped with 2 mg /( kg · min) nomal saline through femoral vein. Other groups were intervened with 0. 5% bupivacaine hydrochloride for modeling. Group Ⅰ was injected with 0. 1 m L s aline on bilateral Neiguan( PC6). When arrhythmia occurs,group Ⅱ and group Ⅳ were injected with the same a mount of saline and puerarin respectively on bilateral Neiguan( PC6). Group Ⅲ was injected with puerarin 30 minutes before bupivacaine hydrochloride bumping. Time of arrhythmia and heart arrest was recorded after local anesthesia. The activities of Na^+-K^+-ATPase and lactate dehydrogenase( LDH) in cardiac muscular tissue were tested. Results Compared with group Ⅱ,the onset time of arrhythmia and heart arrest in group Ⅲ and group Ⅳwere obviously later; the dose of bupivacaine hydrochloride increased accordingly; the activity of Na^+-K^+-ATPase increased and that of LDH decreased more clearly. There was a significant difference( P<0.05 or P<0.01). Meanwhile all respects above mentioned showed remarkably most in group Ⅲ,the differences were significant( P < 0.05). Conclusion Puerarin acupoint injection can reduce the cardiac toxicity of bupivacaine hydrochloride in rats.The mechanism may be due to the increase of myocardial Na^+-K^+-ATPase’activity as well as the decrease of myocardial LDH’s activity.
