摘要:目的探讨秦艽不同配伍药对对风湿热痹型类风湿关节炎(RA)大鼠踝关节血管内皮细胞生长因子(VEGF)表达及病理学的影响。方法 SD大鼠80只,随机分为正常对照组、Ⅱ型胶原组、病证模型组、阳性药组(雷公藤多苷片组,6 mg·kg^(-1))、单味秦艽组、秦艽威灵仙组(平-温相配)、秦艽桑寄生组(平-平相配)和秦艽防己组(平-寒相配),每组10只;采用天然牛Ⅱ型胶原(CⅡ)+人工气候箱模拟人类痹证成因,连续造模18 d建立风湿热痹型RA模型大鼠;造模结束后,各中药组按25.05 g·kg^(-1)连续灌胃给药21 d。实验第38天对各组大鼠进行关节炎指数(AI)评分;HE染色观察踝关节组织病理切片;分别用免疫组织化学法及Western Blot法检测踝关节VEGF的表达。结果与正常对照组比较,Ⅱ型胶原组和病证模型组踝关节滑膜组织血管增生,结缔组织破坏,炎性细胞浸润,血管翳形成,软骨破坏,且病证模型组关节面完全破坏,其破坏程度明显高于Ⅱ型胶原组;Ⅱ型胶原组和病证模型组AI评分、VEGF表达量均显著增加(P<0.01),且病证模型组高于Ⅱ型胶原组。与病证模型组比较,各给药组炎性细胞明显减少,毛细血管减少,绒毛变少,肉芽组织老化,纤维增多成为瘢痕,并且软骨破坏减轻;各给药组的AI评分、VEGF表达量均有不同程度的降低,其中秦艽防己组最为显著(P<0.01)。结论秦艽各配伍药对可能通过调节风湿热痹型RA模型大鼠的VEGF表达,进而抑制新生血管的增生,发挥对RA的治疗作用,且平-寒相配效果优于平-温、平-平相配。
Objective To explore effects of Qinjiao(Gentianae macrophyllae Pall.) in different herb pair combinations on the ankle pathological changes and the expression of VEGF in wind-damp-heat arthralgia RA model rats. Methods Eighty SD rats were randomly divided into normal control group,type Ⅱ collagen group,disease syndrome(wind-damp-heat arthralgia model) group,positive drug group(tripterygium glycosides group,6 mg·kg^(-1)),single Qinjiao group,Qinjiao-Weilingxian(mild-warm compatibility) group,Qinjiao-Sangjisheng(mildmild compatibility) group,Qinjiao-Fangji(mild-cold compatibility) group,ten in each group. Using natural cow type Ⅱ collagen(CⅡ) + artificial climate chamber to simulate the cause of human arthralgia syndrome,winddamp-heat arthralgia RA model rats were established by continuous modeling for 18 days. After modeling,rats in Chinese medicine group were gavaged for 21 days continuously at dose of 25.05 g · kg^(-1). On the 38^(th) day of the experiment, arthritis index(AI) score of each group was evaluated. HE staining of the pathological changes of ankle joint were observed. The expression of VEGF protein in the ankle joint of rats was detected by immunohistochemistry and Western Blot. Results HE staining results showed that, compared with the normal control group, the proliferated vascular of synovial tissue, destruction of connective tissue, inflammatory cell infiltration, angiogenesis, damaged cartilage were obvious in the ankle of type Ⅱ collagen group and disease syndrome model. The joint surface of the disease syndrome model group was destroyed completely,the degree of destruction was significantly higher than type Ⅱ collagen group. AI score and VEGF expression in type Ⅱ collagen group and the disease syndrome model group were significantly increased(P〈0.01),and in the disease syndrome model group were higher than in type Ⅱ collagen group. Compared with the disease syndrome model group,the inflammatory cells were significantly decreased, the capillaries decreased, the number of villi decreased, the granulation tissues aged,the fibrosis increased,and the cartilage destroyed,the AI scores and the expression of VEGF all reduced with different degrees intreated groups, of which Qinjiao-Fangji group was reduced most significantly(P〈0.01) among all groups. Conclusion Different compatibility of Qinjiao may adjust expression of VEGF in wind-damp-heat arthralgia RA model rats and inhibit the proliferation of new blood vessels to exert therapeutic effect for RA,of which effect compatibility of mild-cold is better than mild-warm,mild-mild.
