摘要:旨在从玄府-脑络与神经血管单元(Neurovascular unit,NVU)相关性分析脑小血管病(Cerebral small vessel disease,CSVD)的病机治法。现有研究证实,NVU稳态失衡构成CSVD的核心病理特征,其涉及神经递质调控异常、免疫应答失调、炎症级联激活、氧化应激失衡及其相互作用,较为复杂。玄府-脑络和NVU分别是传统医学和现代医学理论中的微观结构,两者具有极大相似性。本文从结构特性、生理功能进行相关性分析,发现“玄府闭阻,脑络失养”与NVU失稳态具有共性内涵,进而从玄府-脑络理论探讨CSVD的病机治法。CSVD病位在脑,病变在玄府,发病过程分为三个阶段,早期玄府失司,气络郁阻,中期虚实夹杂,郁虚并存,后期肾虚髓减,脑络失养。在治疗中当以调畅玄府,祛邪通络、补肾填精,开玄荣络为原则。本文通过将玄府-脑络与NVU相类比,在看似独立的发病机制中寻找相同致病环节,以期为CSVD等难治性疾病提供安全有效的中西医结合治疗方案。
This article aims to analyze the pathogenesis and treatment principles of cerebral small vessel disease(CSVD)from the perspective of the correlation between the Xuanfu-brain collaterals theory and the neurovascular unit(NVU).Existing studies have shown that the imbalance of NVU homeostasis constituted the core pathological feature of CSVD,which involved abnormal regulation of neurotransmitters,disordered immune response,activation of inflammatory cascades,imbalance of oxidative stress,and their interactions,thus rendering the pathological mechanism of CSVD relatively complex.Xuanfu-brain collaterals and NVU are microscopic structures in traditional Chinese medicine and modern medical theories respectively,and they share great similarities.Therefore,this article conducts a correlation analysis from the aspects of structural characteristics and physiological functions,and holds that“Xuanfu obstruction and insufficient nourishment of brain collaterals”and NVU homeostasis imbalance have common connotations.Furthermore,it explores the pathogenesis and treatment principles of CSVD from the theory of Xuanfu-brain collaterals,and argues that the disease location of CSVD is in the brain,and the lesion is in Xuanfu.The pathogenesis process is divided into three stages:in the early stage,Xuanfu loses its normal function and Qi collaterals are stagnated;in the middle stage,there is a mixture of deficiency and excess,with both stagnation and deficiency;in the late stage,kidney deficiency leads to marrow reduction and insufficient nourishment of brain collaterals.In treatment,the principles should be to regulate and smooth Xuanfu,eliminate pathogenic factors and dredge collaterals,tonify the kidney and replenish essence,as well as open Xuanfu and nourish collaterals.To provide safe and effective integrated traditional Chinese and Western medicine treatment schemes for refractory diseases such as CSVD,the same pathogenic links were sought in the seemingly independent pathogenesis by comparing Xuanfu-brain collaterals with NVU.
