摘要：耳鸣是临床常见病与多发病,是患者自我感觉耳内发出响动,但外界并无声响,其声音或如蝉鸣声或如哨声,大小不一。近年来随着社会环境的改变,生活压力的增加,耳鸣患病率普遍升高,青年患者也越来越多。西医认为耳鸣的产生主要因耳蜗、听觉中枢病变或神经传导路径改变,但具体发病机制尚不明确,治疗主要是营养耳部神经、改善耳部的血液循环、选择佩戴助听器等,但治疗费用大、疗效不甚满意。中医认为本病总属耳脉闭塞,耳窍失养,发为耳鸣,中医将本病分为风邪侵袭证、肝火上扰证、痰火郁结证、肾精亏损证、脾胃虚弱证5个证型。临床治疗多从肝肾论治,虽取得了较好的疗效,但仍对一部分患者效果甚微。而脾胃为后天之本,受纳水谷以化生气、血、津液,濡养五脏六腑及四肢九窍,脾胃气血充盈则耳窍通利。由此可见,脾胃在耳鸣的发生发展及预后中至关重要。中医治疗耳鸣主要采用针灸、中药汤剂、音乐疗法等,其中针灸治疗脾胃虚弱型耳鸣有操作简便、疗效显著、无不良反应的优势。笔者通过查阅分析中国知网、维普、万方临床最新相关文献,从脾胃论治耳鸣的理论依据及针灸治疗进展方面对从脾胃论治耳鸣的临床研究现状加以总结,以期为针灸治疗耳鸣提供参考。
Tinnitus is a common clinical disease and frequently-occurring disease, and it is the patient's self-feeling of ear sounds, but there is no sound in the outside world, it may sound as cicadas or whistles, of which the size is different. In recent years, with the change of social environment, the increase of life pressure, the prevalence of tinnitus is generally increasing, and more and more young patients are also suffering from it. Western medicine believes that tinnitus is mainly caused by cochlea, auditory center disease or nerve conduction pathway changes, but the specific pathogenesis is not clear, the main treatment is to nourish the ear nerve, improve the ear blood circulation, choose to wear hearing AIDS, etc., but the treatment cost is large, the effect is not satisfactory. Traditional Chinese medicine believes that the disease is a total of ear and vein occlusion, resulting in the loss of the ear nourishing, which leads to the onset of tinnitus. Traditional Chinese medicine divides the disease into 5 syndrome types of wind evil invasion, liver fire disturbance, phlegm fire stagnation, kidney essence deficit, and spleen and stomach weakness. Clinical treatment is mostly based on liver and kidney treatment, although it has achieved good curative effects, it still has little efficacy on some patients. The spleen and stomach is the foundation of acquired constitution, receiving food essence to transform Qi( 气), blood and body fluid, nourish the five viscera and the nine orifices of the limbs, and if the spleen and stomach Qi and blood filling the ear orifices is sufficient, the hearing will be normal. Therefore, the spleen and stomach play an important role in the development and prognosis of tinnitus. Traditional Chinese medicine treatment of tinnitus mainly includes acupuncture, traditional Chinese medicine decoction, music therapy, among which acupuncture has the advantages of simple operation, significant efficacy, no adverse reaction in the treatment of tinnitus of spleen and stomach dificiency type. Based on the review and analysis of the latest relevant clinical literature in CNKI, VIP, and Wanfang, this paper summarizes the current clinical research status of the treatment of tinnitus based on the spleen and stomach from the aspects of theoretical basis and progress of acupuncture treatment, in order to provide reference for the treatment of tinnitus by acupuncture.
