摘要:目的:观察敦煌含噙乌梅生脉饮治疗维持性血液透析气阴两虚证患者口干口渴症状的临床疗效及对唾液生化各项离子指标的影响。方法:纳入72例甘肃中医药大学附属医院肾病科血液透析中心有口渴症状的维持性血液透析患者,同时符合气阴两虚证的临床辨证,采用随机数字表法分为观察组(基础治疗加含噙乌梅生脉饮)和对照组(基础治疗加含噙温开水)各36例,以1周为1疗程,观察2个疗程。运用填写量表及客观测量等形式收集整理基本信息及各项指标,以XQ评分表来评定口干口渴情况,采用棉签称重的方法客观测量唾液分泌—静息唾液流率,化学分析仪测量唾液生化Cre、Urea、Na、K、Ca水平。结果:治疗前观察组与对照组基线资料比较无统计学意义(P>0.05);治疗后XQ评分、静息唾液流率UWS,观察组明显优于对照组(P<0.05),而口渴程度分级及唾液生化Cre、Urea、Na、K、Ca水平未见明显异常(P>0.05)。结论:敦煌含噙乌梅生脉饮对维持性血液透析患者改善口渴症状有一定效果,可明显降低口渴XQ评分,增加唾液腺分泌,减少液体摄入,对口渴程度分级及唾液生化未见明显影响。
Objective:To observe the clinical effect of Dunhuang Wumei Shengmai Yin on thirst and dry mouth and its influence to saliva biochemical ion indexes in the treatment of maintenance hemodialysis(MHD)of deficiency of qi and yin.Methods:72 cases of MHD with thirst symptoms,were randomly divided into the observation group and the control group,with 36 cases in each group.The observation group was treated with gargle of Wumei Shengmai Yin,and the control group was treated with gargle of warm water when the patients felt thirsty.One week was taken as one course of treatment and two courses of treatment were observed.Basic information and various indicators were collected and sorted out by means of filling in scales and objective measurements,XQ score sheets was used to assess dryness and thirst,saliva secretion-resting saliva flow rate was objectively measured by using cotton swab weighing method,and the saliva biochemical levels of Creatinine,Urea,Na,K and Ca were detected with chemical analyzer.Results:There was no statistical difference in the baseline between the observation group and the control group before the treatment(P>0.05).The XQ score and unstimulating whole saliva(UWS)flow rate were significantly improved in the observation group than those in the control group after the treatment(P<0.05);whereas the grade of thirst and saliva biochemistry showed no significance between the two groups after the treatment(P>0.05).Conclusion:Dunhuang Wumei Shengmai Yin has a certain effect on the improvement of thirst symptoms in patients with MHD.It can significantly reduce the thirst XQ score,increase salivary gland secretion and reduce fluid intake,but it has no significant effect on the grade of thirst and saliva biochemical test.
