摘要：肛肠疾病术后创面因解剖位置特殊,易受污染且愈合缓慢,传统疗法效果有限。中医“煨脓长肉”理论认为可通过调节创面微环境,以祛腐生肌为核心,促进愈合。甘草归蜡膏源于《赵炳南临床经验集》,具有祛脓长肉、和血生肌的功效,兼具抗炎、促修复及微环境调控作用。其机制包括抑制炎症期过度反应,调控巨噬细胞M1/M2极化平衡,促进血管生成因子表达,加速肉芽组织形成及上皮再生。文章基于“煨脓长肉”理论对甘草归蜡膏治疗肛肠术后创面的应用现状及机制进行综述。
Postoperative wounds of anorectal diseases are prone to contamination and slow healing due to their special anatomical location,and the efficacy of traditional therapies is limited.The traditional Chinese medicine(TCM)theory of Weinong Zhangrou(mellowing pus to promote granulation)takes removing necrotic tissue and promoting granulation as the core,and accelerates wound healing by regulating the wound microenvironment.Gancao Guila Ointment,which is derived from A Collection of Zhao Bingnan's Clinical Experience,has the effects of eliminating pus,promoting granulation,harmonizing blood and regenerating muscle,as well as the functions of anti-inflammation,promoting tissue repair and regulating microenvironment.Its mechanisms include inhibiting excessive inflammatory response in the inflammatory phase,regulating the polarization balance of M1/M2 macrophages,promoting the expression of angiogenesis factors,accelerating the formation of granulation tissue and epithelial regeneration.This article reviews the application status and mechanism of Gancao Guila Ointment in the treatment of postoperative wounds of anorectal diseases based on the theory of Weinong Zhangrou.
