摘要:腰椎间盘疾病是因腰椎间盘产生原发性或继发性病理改变引发一系列临床症状的疾病。原发性改变包括椎间盘退变、膨出、突出等引起的临床症状,包括终板炎及许莫氏结节病。而继发性则因椎间盘突出导致的椎管及神经根管狭窄等疾病。其发病率在全球范围内呈逐年上升趋势,严重影响人类健康与生活质量。富血小板血浆(PRP)治疗作为一种新兴的生物治疗方法,近年来在腰椎间盘疾病的治疗中颇受关注。PRP富含多种生长因子,如血小板源性生长因子(PDGF)、转化生长因子-β(TGF-β)、胰岛素样生长因子(IGF)、血管内皮生长因子(VEGF)等,其在组织修复和再生过程中发挥着关键作用,能够促进细胞的增殖、分化和迁移,刺激细胞外基质的合成,增强组织的修复能力,从而为腰椎间盘疾病的治疗提供新的思路和方法。本文就其促进椎间盘修复与再生机制以及临床转归进行文献综述。
Lumbar intervertebral disc disease is a series of clinical symptoms caused by primary or secondary pathologic changes in the lumbar intervertebral disc.Primary changes include clinical symptoms caused by disc de?generation,bulging and herniation,including endplate inflammation and Schmorl’s nodes.Secondary changes in?clude herniated discs that cause narrowing of the spinal canal and nerve root canal.Its incidence rate is increasing year by year in the world,which seriously affects human health and quality of life.Platelet-rich plasma(PRP)therapy,as a new biological therapy method,has attracted much attention in the treatment of lumbar disc diseases in recent years.PRP is rich in various growth factors,such as platelet-derived growth factor(PDGF),transforming growth factor-β(TGF-β),insulin-like growth factor(IGF),vascular endothelial growth factor(VEGF)and so on,which plays a key role in the process of tissue repair and regeneration,promoting the proliferation,differentiation and migration of cells,stimulating the synthesis of extracellular matrix,and enhancing the ability of tissue repair,thus providing new ideas and methods for the treatment of lumbar disc diseases.In this paper,the mechanism and clinical outcome of promoting intervertebral disc repair and regeneration are reviewed.
