摘要:目的:研究甘肃省医疗卫生资源失配度的时空格局及影响因素,为医疗卫生资源的高效配置提供参考。方法:基于2012-2021年甘肃省面板数据,采用健康距离模型与岭回归方法,分析甘肃省医疗卫生资源失配度的时空格局及影响因素。结果:甘肃省医疗卫生资源失配度均值从2012年的0.732降低至2021年的0.636。2012-2013年失配度高(较高)的市(州)占比达50%,其后逐年减少,至2021年失配度低(较低)的市(州)占比增至57.14%。城镇化率、一般公共预算收入与支出的比值、人口规模、人口密度及人口老龄化指标岭回归系数小于0(P<0.001)。结论:2012-2021年甘肃省医疗卫生资源失配度不断降低,但至2021年仍处于一般失配状态。时空格局上呈现出从“高度失配主导的非均衡”到“西北>东南”空间格局,再到“中部低水平失配团状集聚”的演进趋势。城镇化率、人口密度、人口规模等指标的提升有利于降低医疗卫生资源失配度。
Objective:To study the spatial-temporal characteristics and influencing factors of the mismatch of medical and health resources in Gansu Province,providing a valuable reference for the efficient allocation of medical and health resources.Methods:Based on panel data of Gansu Province from 2012 to 2021,the health distance model and ridge regression method were used to analyze the spatial-temporal characteristics and influencing factors of the mismatch of medical and health resources.Results:The mean mismatch degree of medical and health resources in Gansu Province decreased from 0.732 in 2012 to 0.636 in 2021.From 2012 to 2013,the proportion of high(higher)level units reached 50%,then decreased year by year,and the number of low(lower)level units increased by 57.14%in 2021.The ridge regression coefficients of population urbanization rate,the ratio of general public budget revenue to expenditure,population size,population density,and population aging indexes were all less than 0(P<0.001).Conclusion:The mismatch degree of medical and health resources continues to decrease and was still in a general mismatch state by 2021.In terms of regional space,there is an evolving situation of disequilibrium dominated by high mismatch to spatial difference in northwest>southeast,and then to low-level mismatch cluster in central China.Population urbanization,population density,population size,and other variables help reduce the mismatch of medical and health resources.
