摘要：目的 观察生肌玉红膏联合负压封闭引流术(VSD)治疗糖尿病足溃疡的临床疗效。方法 将30例糖尿病足溃疡患者采用随机数字表法分为对照组和治疗组,各15例。对照组给予VSD治疗,治疗组在对照组治疗方法的基础上给予生肌玉红膏治疗,2组均治疗28 d。比较2组病原菌分布情况、肉芽组织生长时间,治疗后第14,28天创面愈合率,治疗前后白细胞计数,中性粒细胞百分比,白蛋白、球蛋白含量,血清白细胞介素-6(IL-6)、降钙素原(PCT)、C-反应蛋白(CRP)水平,以及随访1年的复发率、截肢率及病死率。结果 2组病原菌分布情况比较,差异均无统计学意义(P>0.05),均以金黄色葡萄球菌为主。与同组治疗后第14天比较,2组治疗后第28天创面愈合率明显升高,差异均有统计学意义(P<0.01)。治疗组新生肉芽组织生长时间为(10.93±2.76) d,对照组为(14.20±2.86) d, 2组比较差异有统计学意义(P<0.05)。治疗后第14,28天2组白细胞计数、中性粒细胞百分比及白蛋白、球蛋白含量均明显降低,与同组治疗前比较差异有统计学意义(P<0.05或P<0.01),且治疗后第28天治疗组白蛋白、球蛋白含量降低更明显,与对照组同期比较差异均有统计学意义(P<0.01)。治疗后第14,28天2组血清IL-6、PCT、CRP水平均明显降低,与同组治疗前比较差异有统计学意义(P<0.05或P<0.01),且治疗组降低更明显,与对照组同期比较差异均有统计学意义(P<0.05)。治疗组复发率为13.3%(2/15),截肢率和病死率均为0;对照组复发率为40.0%(6/15),截肢率和病死率均为6.7%(1/15),2组复发率、截肢率及病死率比较差异均有统计学意义(P<0.05)。结论 生肌玉红膏联合VSD治疗糖尿病足溃疡可缩短肉芽组织生长时间,减轻炎症反应,降低复发率、截肢率及病死率,值得临床推广应用。
Objective To observe the clinical efficacy of Shengji Yuhong Ointment(生肌玉红膏)combined with vacuum sealing drainage(VSD)in the treatment of diabetic foot ulcers.Methods 30 patients with diabetic foot ulcer were divided into control group and treatment group according to random number table method,15 cases in each group.The control group was treated with VSD,and the treatment group was treated with Shengji Yuhong Ointment on the basis of the treatment method of the control group.Both groups were treated for 28 days.The distri-bution of pathogenic bacteria,the growth time of granulation tissue,the wound healing rate on the 14th and 28th day after treatment,the white blood cell count,the percentage of medium and fine granulocytes,the contents of albumin and globulin before and after treatment,and the contents of interleukin-6(IL-6),procalcitonin(PCT),and C-reac-tive protein(CRP)in serum were compared between the two groups.The recurrence rate,amputation rate and mor-tality rate were followed up for 1 year.Results There was no significant difference in the distribution of pathogenic bacteria between the two groups(P>0.05).Staphylococcus aureus was the main pathogen in both groups.Com-pared with the same group on the 14th day after treatment,the wound healing rate of the two groups on the 28th day after treatment was significantly higher,and the differences were statistically significant(P<0.01).The growth time of new granulation tissue was(10.93±2.76)d in the treatment group and(14.20±2.86)d in the control group,and the difference between the two groups was statistically significant(P<0.05).On the 14th and 28th day after treatment,the white blood cell count,neutrophil percentage,albumin and globulin contents of the two groups were significantly decreased,and the differences were statistically significant compared with those before treatment(P<0.05 or P<0.01),and the albumin and globulin contents of the treatment group were more significantly de-creased on the 28th day after treatment.Compared with the control group,the difference was statistically significant(P<0.01).Serum IL-6,PCT and CRP levels in the 2 groups were significantly decreased on the 14th and 28th day after treatment,with statistical significance compared with the same group before treatment(P<0.05 or P<0.01),and the reduction was more obvious in the treatment group,with statistical significance compared with the control group at the same period(P<0.05).In the treatment group,the recurrence rate was 13.33%(2/15),and the am-putation rate and mortality were 0.In the control group,the recurrence rate was 40.0%(6/15),and the amputation rate and mortality were 6.7%(1/15).There were statistically significant differences in the recurrence rate,amputa-tion rate and morta-lity between the two groups(P<0.05).Conclusion Shengji Yuhong Ointment combined with VSD in the treatment of diabetic foot ulcers can accelerate the growth of granulation tissue,reduce inflammatory re-action,reduce the recurrence rate,amputation rate and mortality,which is worthy of clinical application.
