摘要：目的观察丝线结扎在复杂困难型胆囊炎患者腹腔镜胆囊切除术中的应用效果。方法将68例复杂困难型胆囊炎患者采用随机数字表法分为2组,各34例。2组均行腹腔镜胆囊切除术,术中对照组采用可吸收生物夹结扎胆囊管和胆囊动脉,治疗组采用丝线结扎,2组术后均给予常规治疗。比较2组术中出血量、手术时间、拔除引流管时间及住院费用,以及术后出血、胆漏、胆管狭窄、膈下感染及粘连性肠梗阻等并发症发生情况。结果2组术中出血量、手术时间及拔除引流管时间比较,差异均无统计学意义(P>0.05);治疗组住院费用明显低于对照组,2组比较差异有统计学意义(P<0.01);2组术后均未发生出血、胆漏、胆管狭窄、膈下感染和粘连性肠梗阻等并发症。结论对于复杂困难型胆囊炎患者,在腹腔镜胆囊切除术中采用丝线结扎胆囊管及胆囊动脉安全可靠,并可降低住院费用。
Objective To observe the application effects of silk thread ligation in laparoscopic cholecystectomy for patients with complicated and difficult cholecystitis.Methods Sixty-eight cases with complicated and difficult cholecystitis were divided into two groups according to random number table method,with 34 cases in each.The 2 groups were both given laparoscopic cholecystectomy,during the operation,the absorbable biological clip was used to ligate cystic duct and cystic artery in the control group,silk thread ligation was used in the treatment group,and routine treatment was given to both groups after operation.Intraoperative blood loss,operation time,drainage tube removal time,hospitalization expenses,the incidence of the postoperative complications such as bleeding,bile leakage,bile duct stricture,subphrenic infection and adhesive intestinal obstruction between the two groups were compared.Results There was no significant difference in intraoperative blood loss,operation time and drainage tube removal time between the two groups(P>0.05);the hospitalization expenses in the treatment group were significantly lower than those in the control group,and the difference between the 2 groups was statistically significant(P<0.01);there were no postoperative complications such as bleeding,bile leakage,biliary duct stricture,subphrenic infection and adhesive intestinal obstruction in both groups.Conclusion For patients with complicated and difficult cholecystitis,silk thread ligation of cystic duct and cystic artery in laparoscopic cholecystectomy is safe and reliable,and can reduce the hospitalization expenses.
