摘要：目的:观察芪归养骨方联合体外冲击波治疗ARCOⅠ期、ARCOⅡ期激素性股骨头坏死的近期疗效。方法:收集41例股骨头坏死患者,随机分为治疗组21例(41髋),对照组20例(38髋),治疗组采用芪归养骨方联合体外冲击波疗,对照组采用阿仑膦酸钠和碳酸钙D3片保守治疗,比较两组患者治疗前后疼痛(VAS)及功能(Harris)评分变化。结果:两组患者的VAS评分及Harris评分治疗后较治疗前均有改善(P<0.05),治疗组的VAS及Harris前后评分较对照组改善更明显(P<0.05)。观察组总有效率更高(P<0.05),不良反应发生率更低(P<0.05)。结论:运用芪归养骨方联合冲击波综合治疗ARCOⅠ期、ARCOⅡ期激素性股骨头坏死在改善患者髋关节功能、减轻患者的疼痛、阻止股骨头坏死进一步塌陷方面,短期临床疗效较单纯内服药物治疗效果明显,患者依从性好、满意度高,值得临床推广应用。
Objective:to observe the Astragalus and Angelica Mixture formulae Combined with extracorporeal shock wave therapy Steroid induced necrosis of femoral head avascular necrosis(ARCOⅠ,Ⅱ)of the clinical short-term efficacy.Methods:41 cases have come from the outpatient of joint orthopedics and traumatology and Xi Gu orthopedics and traumatology.The treatment group was treated with Astragalus and Angelica Mixture formulae combined with shock wave therapy,and the control group was treated conservatively with alendronate sodium and D3 calcium carbonate tablets for 12 weeks.The VAS and Harris scores before and after treatment in the two groups were compared.Results:VAS score and Harris score after treatment in both groups were improved compared with that before treatment(P<0.05).However,VAS score and Harris score after treatment in the treatment group were more improved than that in the control group(P<0.05).Compared with the control group,the total effective rate in the observation group was significantly increased(P<0.05)and the incidence of adverse reactions was significantly reduced(P<0.05).Conclusion:Comprehensive treatment of Astragalus and Angelica Mixture formulae combine with shock waves is applied ARCOⅠ,Ⅱperiod in improving the hip joint function in patients with hormone osteonecrosis,ease the pain of patients,to prevent further collapse in femoral head necrosis,short-term clinical curative effect than pure internal and drug treatment effect is obvious,good patient dependency,satisfaction is high.It is worthy of clinical application.
