摘要：目的探讨扩散加权成像(DWI)及表观扩散系数(ADC)对肾透明细胞癌及非透明细胞癌的鉴别诊断价值。方法回顾性分析100例经病理检查证实为肾细胞癌患者的影像资料(包括DWI及ADC),其中肾非透明细胞癌32例(包括21例嫌色细胞癌、11例乳头状细胞癌),肾透明细胞癌68例。所有患者均给予DWI(包括高、低b值DWI及ADC)比较2组影像学特征,并测量肿瘤部位的ADC值。结果DWI及ADC在2组对比差异有统计学价值(P<0.05);透明细胞癌DWI多为低、等或稍高信号,ADC多为等或稍高信号,并且高b值DWI信号低于低b值DWI;非透明细胞癌DWI多为明显高信号,ADC多为较均匀的明显低信号,并且高b值DWI信号明显高于低b值DWI,非透明细胞癌ADC值比透明细胞癌ADC值低,ADC值<(1.42±0.48)×10^(-3)mm^(2)/s大多为非透明细胞癌。结论肾透明细胞癌高b值DWI信号低于低b值DWI信号,ADC为等或稍高信号,非透明细胞癌高b值DWI信号高于低b值DWI信号,ADC为均匀明显低信号的影像特点对肾透明细胞癌及非透明细胞癌有很大的鉴别诊断价值。
Objective To evaluate the value of diffusion weighted imaging(DWI)and apparent diffusion coefficient(ADC)in the differential diagnosis of renal clear cell carcinoma and non-clear cell carcinoma.Methods The imaging data of 100 patients with renal cell carcinoma(including DWI and ADC)confirmed by pathology were analyzed retrospectively,including 32 cases of renal non-clear cell carcinoma(including 21 cases of chromophobe cell carcinoma,11 cases of papillary cell carcinoma)and 68 cases of renal clear cell carcinoma.All the selected patients were given DWI(including high and low b value DWI and ADC)to compare the imaging features of the two groups,and measure the ADC value of the tumor site.Results There was significant difference in DWI and ADC between the two groups(P
