摘要:目的观察平痫冲剂及其拆方对幼龄癫痫模型大鼠的抗痫作用与脑皮质区超微结构及相关蛋白基质金属蛋白酶-9(MMP-9)、水通道蛋白-4(AQP-4)表达的影响。方法SPF级28日龄Wistar大鼠共450只,随机选取60只作为空白组,造模组390只,采用戊四唑腹腔注射制备癫痫模型。模型制备成功的360只大鼠随机分成模型组、西药组、化痰组、祛瘀组、息风组及平痫冲剂组,每组60只,分别予以药物灌胃21 d;各组大鼠随机选取10只,分别于干预24 h、48 h、72 h、7 d、14 d、21 d后,利用戊四唑腹腔注射诱导,观察1 h内各组大鼠惊厥发作情况,并取大鼠脑组织,进行皮质区超微结构观察及MMP-9、AQP-4表达的变化。结果与模型组比较,西药组在用药干预24 h、48 h MMP-9阳性表达减弱(P<0.05或P<0.01);化痰组在用药干预14 d MMP-9阳性表达减弱(P<0.05);息风组在用药干预7 d MMP-9阳性表达减弱(P<0.05);平痫冲剂组在用药干预24 h MMP-9阳性表达减弱(P<0.05),48 h作用最显著(P<0.01)。与同组用药干预24 h后比较,西药组在用药干预72 h和7 d MMP-9阳性表达减弱(P<0.05或P<0.01);化痰组在用药干预14 d MMP-9阳性表达减弱(P<0.05),至21 d MMP-9蛋白阳性表达显著减弱(P<0.01);平痫冲剂组在用药干预72 h MMP-9阳性表达明显减弱(P<0.01)。与各拆方组比较,平痫冲剂组在用药48 h后各时间段MMP-9表达均明显减弱(P<0.01)。与模型组比较,西药组在用药干预24 h、48 h AQP-4阳性表达减弱(P<0.05或P<0.01);化痰组在用药干预14 d AQP-4阳性表达减弱(P<0.05);息风组在用药干预72 hAQP-4阳性表达减弱(P<0.05);平痫冲剂组在用药干预48 h AQP-4阳性表达减弱(P<0.05),72 h作用最为显著(P<0.01)。与同组用药干预24 h比较,西药组在用药干预48 h、72 h AQP-4阳性表达减弱(P<0.05或P<0.01);平痫冲剂组在用药干预72 h AQP-4阳性表达明显减弱(P<0.01)。与各拆方组比较,平痫冲剂组在用药干预7 d AQP-4阳性表达减弱(P<0.05),至14 d AQP-4阳性表达显著减弱(P<0.01)。结论平痫冲剂及其拆方均具有一定的抗癫痫作用,随着用药干预时间的延长,癫痫模型大鼠脑皮质神经元细胞核病理改变及突触结构病理改变也较前明显减轻,且MMP-9、AQP-4阳性表达降低,其中以平痫冲剂作用最为显著。
Objective To observe the antiepileptic effects of Pingxian Chongji(平痫冲剂)along with decomposed prescriptions thereof and effects on expression on young epileptic model rats and effects on cerebral cortical area ultrastructure and expression of related proteinsmatrix metallopeptidase-9(MMP-9)and aquaporin-4(AQP-4).Methods Among 450 SPF Wistar rats aged 28 days,60 were randomly chosen as blank group,and 390 rats as establishing model group.The epilepsy model was prepared by intraperitoneal injection of pentetrazole.After the model was successfully established,the rats were randomly divided into model group,Western medicine group,phlegm resolving group,blood stasis removing group,stopping wind group and Pingxian Chongji group,with 60 rats in each group.They were given drugs by gavage for 21 days respectively.Ten rats were randomly chosen from each group,and were induced by intraperitoneal injection of pentetrazol after 24 hours,48 hours,72 hours,7 days,14 days and 21 days,respectively,the seizure attacks of rats in each group were observed within 1 hour,and the brain tissues of rats were taken to observe cortex area ultrastructureand expression of MMP-9 and AQP-4.Results Compared with the model group,the positive expression of MMP-9 in Western medicine group decreased at 24 hours and 48 hours after medication intervention(P<0.05 or P<0.01);the positive expression of MMP-9 in phlegm resolving group decreased on the 14 th day after medication intervention(P<0.05);The positive expression of MMP-9 in stopping wind group decreased on the 7 th day after medication intervention(P<0.05);The positive expression of MMP-9 in Pingxian Chongji group decreased at 24 hours after medication intervention(P<0.05),and the effect was the most significant at 48 hours(P<0.01).Compared with the same group at 24 hours after medication intervention,the positive expression of MMP-9 in Western medicine group decreased at 72 hours and on the 7 th day after medication intervention(P<0.05 or P<0.01);the positive expression of MMP-9 in phlegm resolving group decreased on the 14 th day after medication intervention(P<0.05)and significantly decreased on the 21 st day after medication intervention(P<0.01);The positive expression of MMP-9 in Pingxian Chongji group significantly decreased at 72 hours after medication intervention(P<0.01).Compared with each decomposed prescriptions group,the expression of MMP-9 in Pingxian Chongji group significantly decreased at each time period after 48 hours of medication intervention(P<0.01).Compared with the model group,the positive expression of AQP-4 in Western medicine group decreased at 24 hours and 48 hours after medication intervention(P<0.05 or P<0.01);the positive expression of AQP-4 in phlegm resolving group decreased on the 14 th dayafter medication intervention(P<0.05);The positive expression of AQP-4 in stopping wind group decreased at 72 hoursafter medication intervention(P<0.05);The positive expression of AQP-4 in Pingxian Chongji group decreased at 48 hoursafter medication intervention(P<0.05),with the most significant effect at 72 hours of medication intervention(P<0.01).Compared withthe same group at 24 hours after medication intervention,the positive expression of AQP-4 in Western medicine group significantly decreased at 48 hours and 72 hours after medication intervention(P<0.05 or P<0.01);the positive expression of AQP-4 in Pingxian Chongji group significantly decreased at 72 hours of medication intervention(P<0.01).Compared with each decomposed prescriptions group,the positive expression of AQP-4 inPingxian Chongji group decreased on the 7 th day after medication intervention,significantly decreased on the 14 th day(P<0.01).Conclusion Pingxian Chongjialong withdecomposed prescriptions thereof have some antiepileptic effects.With the prolongation of medication intervention time,the pathological changes of neuron nucleus and synaptic structure in cerebral cortex area of epilepsy model rats were also significantly reduced than before,and the positive expression of MMP-9 and AQP-4 decreased,among which Pingxian Chongji was the most significant.
