摘要：人乳头瘤病毒(human papilloma virus,HPV)特别是HPV16/18持续感染是宫颈癌前病变和宫颈癌发生的“元凶”,是一种球形DNA病毒,主要感染黏膜鳞状上皮和表皮,临床表现为阴道分泌物异常、阴道瘙痒及尖锐湿疣等外生殖器病变,伴宫颈接触性出血和宫颈肥大等。阴道菌群多样性增加和乳杆菌减少是HPV感染的必要条件,重建以乳杆菌为主的阴道微生物群落结构、恢复阴道微生态平衡能够促进HPV的清除,是未来HPV感染及宫颈病变治疗的潜在方法。通过阴道菌群治疗HPV感染的方法包括益生菌和益生元、补充激素、中医药疗法及生活方式干预。文章着重论述阴道菌群与HPV感染的相关性及治疗方法,以期为HPV感染及宫颈病变的诊治提供参考。
The persistent infection of human papillomavirus(HPV)particularly HPV16/18,is the"crime"of cervical precancerous lesions and cervical cancer,it is a spherical DNA virus that mainly infects mucosal squamous epithelium and epidermis.The clinical features were abnormal vaginal secretion,vaginal pruritus,condyloma acuminatum and other reproductive organs lesions,accompanied by cervical contact bleeding and cervical hypertrophy.In recent years,multiple studies have exhibited that the increase of vaginal flora variety and the decrease of Lactobacillus are the essential conditions for HPV infection,reconstructing the vaginal microbial community structure dominated by Lactobacillus and restoring the vaginal micro ecological balance are capable of promoting the clearance of HPV,it is a lurking method for the remedy of HPV infection and cervical lesions next years.The ways of treating HPV infection through vaginal flora include probiotic and sprebiotics,estrogen,traditional Chinese medicine therapy and lifestyle intervention.This thesis focuses on the correlation and treatment between vaginal flora and HPV infection to provide reference for the diagnosis and treatment of HPV infection and cervical lesions.
