摘要：目的:通过网状Meta分析系统性评价不同艾灸方法对脊髓损伤后神经源性膀胱病人排尿功能的影响。方法:计算机检索中国知网、万方数据库、维普数据库、中国生物医学文献数据库、PubMed、EMbase和the Cochrane Library搜集相关文献,检索时限为建库至2021年6月20日,由2名研究人员独立进行文献检索、筛选和质量评价,并对结果进行交叉核对。使用RevMan 5.4和Stata 14软件进行数据分析。结果:最终纳入25篇临床随机对照试验(RCT),涉及8种不同的艾灸方法。网状Meta分析显示:在总有效率方面,温和灸、隔药灸、隔姜灸、热敏灸、温灸箱灸及益元灸联合常规康复护理优于常规康复护理,累计排序曲线下面积(SUCRA)排序前3位的艾灸方法依次为温和灸、隔药灸、温灸箱灸。在膀胱功能平衡状态方面,温灸箱灸、温和灸联合常规康复护理优于常规康复护理;SUCRA排序前3位的艾灸方法依次为温灸箱灸、温和灸、益元灸。在残余尿量方面,温和灸、隔药灸、隔姜灸、雷火灸、热敏灸、温灸箱灸、益元灸、温针灸联合常规康复护理优于常规康复护理;SUCRA排序前3位的艾灸方法依次为温和灸、雷火灸、隔姜灸。结论:现有证据表明,艾灸联合常规康复护理能有效改善脊髓损伤后神经源性膀胱病人的排尿功能,其中温和灸和温灸箱灸可能是最优选择。此外,仍应通过高质量RCT对本研究结果加以论证。
Objective:To systematically evaluate the effect of different moxibustion methods on the urination function of patients with neurogenic bladder after spinal cord injury by network Meta-analysis.Methods:Related articles were searched from CNKI,WanFang Database,VIP,CBM,PubMed,EMbase and the Cochrane Library.The retrieval time was from the establishment of the database to June 20,2021.Two researchers independently conducted literature retrieval,screening and quality evaluation,and cross checked the results.Data analysis was performed by using RevMan 5.4 and Stata 14 software.Results:A total of 25 RCTs were included,involving 8 different methods of moxibustion.Net Meta-analysis concluded that:in terms of overall effective rate,mild moxibustion,medicine separation moxibustion,ginger separation moxibustion,thermosensitive moxibustion,warm moxibustion box moxibustion,Yiyuan moxibustion combined with conventional rehabilitation nursing was better than conventional rehabilitation nursing;The top three moxibustion methods ranked by SUCRA were mild moxibustion,medicine separation moxibustion and warm moxibustion box moxibustion.In terms of the number of cases of balanced bladder function after treatment,warm moxibustion box moxibustion,mild moxibustion combined with conventional rehabilitation nursing were better than conventional rehabilitation nursing.The top three moxibustion methods in the list of SUCRA were warm moxibustion box moxibustion,mild moxibustion and Yiyuan moxibustion.In terms of residual urine volume,mild moxibustion,medicine separation moxibustion,ginger separation moxibustion,thunder fire moxibustion,thermal sensitivity moxibustion,warm moxibustion box moxibustion,Yiyuan moxibustion and warm acupuncture combined with conventional rehabilitation nursing were superior to conventional rehabilitation nursing.The top three moxibustion methods ranked by SUCRA were mild moxibustion,thunder fire moxibustion and ginger separation moxibustion in turn.Conclusions:Current evidence shows that moxibustion combined with conventional rehabilitation nursing can effectively improve the urination function of patients with neurogenic bladder after spinal cord injury.Mild moxibustion and warm moxibustion box moxibustion may be the best choice.In addition,the results of this study should still be verified by high-quality RCTs.
