摘要:目的观察淫羊藿苷(ICA)对激素抵抗型肾病综合征(SRNS)大鼠PODXL/Ezrin及凋亡蛋白Caspase-3、Bax、Bcl-2表达的影响,探讨其减轻足细胞损伤的作用机制。方法采用盐酸阿霉素注射法建立大鼠SRNS模型,将大鼠分为空白组、模型组、激素组、ICA组和联合组,各给药组分别予相应药物灌胃,连续6周。CCB法检测24 h尿蛋白(24-UTP)含量,全自动血液生化分析仪检测血清白蛋白(ALB)、血肌酐(SCr)、血尿素氮(BUN)含量,Masson染色观察肾小球基底膜及足细胞形态结构,Western blot检测肾组织PODXL、Ezrin、Caspase-3、Bax、Bcl-2蛋白表达,TUNEL染色观察肾组织细胞凋亡情况。结果与空白组比较,模型组大鼠体质量减轻,24-UTP含量升高,血清ALB含量减少,SCr、BUN含量增加;肾小球基底膜沉积物增多,足细胞肿胀、变性、增生;肾组织Bcl-2、PODXL、Ezrin蛋白表达降低,Caspase-3、Bax蛋白表达升高,细胞凋亡增加,差异均有统计学意义(P<0.05,P<0.01)。与模型组比较,ICA组和联合组大鼠体质量增加,24-UTP含量减少,联合组大鼠血清ALB含量增加,SCr、BUN含量减少;各给药组大鼠肾小球基底膜及足细胞病理结构有不同程度改善;ICA组和联合组大鼠肾组织PODXL、Ezrin、Bcl-2蛋白表达升高,联合组大鼠肾组织Caspase-3、Bax蛋白表达降低,细胞凋亡减少,差异均有统计学意义(P<0.05,P<0.01)。结论ICA可缓解SRNS足细胞损伤,其机制与抑制足细胞凋亡、恢复足细胞结构及完整性有关。
Objective To observe the effects of icariin(ICA)on PODXL/Ezrin and Caspase-3,Bax and Bcl-2 of rats with steroid resistant nephrotic syndrome(SRNS);To explore the mechanism of reducing podocyte injury.Methods The SRNS rat model was established by radriamycin hydrochloride injection.The rats were divided into blank group,model group,hormone group,ICA group and combination group.The medicated groups were given corresponding drugs by gavage for 6 weeks.The 24-hour urine protein(24-UTP)content was detected by CCB method,Serum albumin(ALB),blood urea nitrogen(BUN),serum creatinine(SCr)were detected by automatic blood biochemical analyzer,the morphological structure of glomerular basement membrane and podocyte were observed by Masson staining,the expressions of PODXL,Ezrin,Caspase-3,Bax and Bcl-2 protein in renal tissue were detected by Western blot,the apoptosis of renal tissue was observed by TUNEL staining.Results Compared with the blank group,the body mass of rats in the model group decreased,the content of 24-UTP increased,the content of serum ALB decreased,the contents of SCr and BUN increased;the glomerular basement membrane deposits increased,podocytes swelled,degenerated and proliferated;the expressions of Bcl-2,PODXL,Ezrin protein in renal tissue decreased,the expressions of Caspase-3 and Bax protein increased,and the apoptosis increased,with statistical significance(P<0.05,P<0.01).Compared with the model group,the body mass of rats in ICA group and combination group increased,the content of 24-UTP decreased,the content of serum ALB in combination group increased,the contents of SCr and BUN decreased;the pathological structures of glomerular basement membrane and podocytes of rats in each treatment group were improved to varying degrees;the expression of PODXL,Ezrin and Bcl-2 protein in renal tissue of ICA group and combination group increased,the expressions of Caspase-3,Bax protein and apoptosis in renal tissue of rats in combination group decreased,with statistical significance(P<0.05,P<0.01).Conclusion ICA can alleviate the podocyte lesion of SRNS,and the mechanism is related to inhibition of podocyte apoptosis and restoration of podocyte structure and integrity.
