摘要:目的分析维生素D与2型糖尿病(type 2 diabetes mellitus,T2DM)伴糖尿病性视网膜病变(diabetic reti?nopathy,DR)发病风险之间的关系。方法便利选取2023年1月—2025年5月甘肃中医药大学第三附属医院收治的98例T2DM患者,依据是否伴有非增殖期DR分为单纯T2DM组(47例)与T2DM伴DR组(51例),对比两组血清25-羟基维生素D、血糖、血脂、炎症因子指标与眼相关指标。依据血清25-羟基维生素D水平将T2DM伴DR组分为正常组、不足组与缺乏组,对比3组上述各项指标。采用Pearson相关系数分析血清25-羟基维生素D与血糖、血脂、炎症因子与眼相关指标的相关性。结果T2DM伴DR组与单纯T2DM组相比,血清25-羟基维生素D、高密度脂蛋白胆固醇水平与视力均更低,空腹血糖、餐后2 h血糖、低密度脂蛋白胆固醇、甘油三酯、总胆固醇、白细胞介素-6、白细胞介素-36水平均更高,眼底渗出物面积、出血斑面积更大,差异均有统计学意义(P均<0.05)。与正常组及不足组相比,缺乏组血清25-羟基维生素D、高密度脂蛋白胆固醇水平与视力均更低,空腹血糖、餐后2 h血糖、低密度脂蛋白胆固醇、甘油三酯、总胆固醇、白细胞介素-6、白细胞介素-36水平均更高,眼底渗出物面积、出血斑面积更大,差异均有统计学意义(P均<0.05)。血清25-羟基维生素D与空腹血糖、餐后2 h血糖、低密度脂蛋白胆固醇、甘油三酯、总胆固醇、白细胞介素-6、白细胞介素-36、眼底渗出物面积、出血斑面积之间呈负相关,与高密度脂蛋白胆固醇呈正相关(P均<0.05);而与视力无相关性(P>0.05)。结论25(OH)D与T2DM患者的血糖、炎症因子指标及眼底渗出物面积、出血斑面积之间存在紧密联系,临床可根据25(OH)D水平辅助判定患者有无发生DR。
Objective To analyze the relationship between vitamin D and the risk of diabetic retinopathy(DR)in type 2 diabetes mellitus(T2DM).Methods A total of 98 patients with T2DM admitted to the Third Affiliated Hospital of Gansu University of Traditional Chinese Medicine from January 2023 to May 2025 were convenient selected.Accord?ing to whether they were accompanied by non-proliferative DR,they were divided into simple T2DM group(47 cases)and T2DM with DR group(51 cases).Serum 25-hydroxyvitamin D,blood glucose,blood lipids,inflammatory factor indicators and eye-related indicators were compared between the two groups.According to the level of serum 25-hy?droxyvitamin D,the T2DM with DR group was divided into normal group,insufficient group and deficient group,and the above indexes were compared among the three groups.Pearson correlation coefficient was used to analyze the cor?relation between serum 25-hydroxyvitamin D and blood glucose,blood lipid,inflammatory factors and eye-related in?dicators.Results Compared with the simple T2DM group,the T2DM with DR group had lower serum 25-hydroxyvita?min D,high-density lipoprotein cholesterol levels and visual acuity,higher fasting blood glucose,2-hour postprandial blood glucose,low-density lipoprotein cholesterol,triglyceride,total cholesterol,interleukin-6,and interleukin-36 levels,and larger fundus exudate area and bleeding spot area,the differences were statistically significant(all P<0.05).Compared with the normal group and the insufficiency group,the levels of serum 25-hydroxyvitamin D,highdensity lipoprotein cholesterol and visual acuity in the deficiency group were lower,the levels of fasting blood glucose,2-hour postprandial blood glucose,low-density lipoprotein cholesterol,triglyceride,total cholesterol,interleukin-6 and interleukin-36 were higher,the area of fundus exudate and the area of bleeding spots were larger,and the differ?ences were statistically significant(all P<0.05).Serum 25-hydroxyvitamin D was negatively correlated with fasting blood glucose,2-hour postprandial blood glucose,low-density lipoprotein cholesterol,triglyceride,total cholesterol,interleukin-6,interleukin-36,fundus exudate area and hemorrhagic spot area,and positively correlated with highdensity lipoprotein cholesterol(all P<0.05).However,there was no correlation with visual acuity(P>0.05).Conclu?sion There is a close connection between 25(OH)D and the blood glucose,inflammatory factor indicators,and the area of fundus exudates and bleeding spots in patients with T2DM.Clinically,the level of 25(OH)D can be used to assist in the diagnosis of whether a patient has developed DR.
