摘要:系统评价高毒力与非高毒力肺炎克雷伯菌菌株毒力基因检出情况。通过计算机检索PubMed、CNKI、The Cochrane Library、Web of Science、WanFang、VIP和Embase数据库,搜索建库至2022年10月公开发表的关于HvKP毒力基因的相关文献,采用Review Manager5.4软件对文献进行荟萃分析(Meta分析)。共纳入30篇文献,均为回顾性研究。共3117株菌株,其中HvKP组1220株菌株、非HvKP组1897株菌株。Meta分析结果显示,HvKP组rmpA(OR=65.38,95%CI(33.92,126.01),P<0.001)、mrkD(OR=1.73,95%CI(1.06,2.84),P=0.03)、aerobactin(OR=148.63,95%CI(43.41,508.90),P<0.001)、rmpA 2(OR=40.35,95%CI(14.79,110.06),P<0.001)、magA(OR=5.32,95%CI(2.98,9.47),P<0.001)、iroNB(OR=21.43,95%CI(12.77,35.96),P<0.001)、alls(OR=3.85,95%CI(2.88,5.16),P<0.001)、kfu(OR=7.05,95%CI(3.74,13.30),P<0.001)、entB(OR=5.64,95%CI(1.35,23.53),P=0.02)、ybts(OR=2.83,95%CI(1.34,5.97),P=0.006)、iucA(OR=51.49,95%CI(3.78,701.27),P=0.003)、iutA(OR=48.56,95%CI(14.41,163.65),P<0.001)、wcaG(OR=8.95,95%CI(5.98,13.39),P<0.001)和peg-344(OR=37.44,95%CI(0.94,1498.69),P=0.05)基因检出率高于非HvKP组(P<0.05)。而在wabG、fimH、uge、ureA基因检出率方面,差异无统计学意义(P>0.05)。与非HvKP组相比,HvKP组rmpA、mrkD、aerobactin、rmpA 2、magA、iroNB、alls、kfu、entB、ybts、iucA、iutA、wcaG和peg-344基因检出率更高,而wabG、fimH、uge、ureA基因检出方面效果相当。
To systematically evaluate the detection rates of virulence genes in hypervirulent(HvKP)and non-hypervirulent Klebsiella pneumoniae(non-HvKP)strains.We conducted a comprehensive search of PubMed,CNKI,The Cochrane Library,Web of Science,Wanfang,VIP,and Embase databases for relevant literature published from the inception of these databases to October 2022.The literature focused on the comparison of virulence gene detection between HvKP and non-HvKP strains.Review Manager 5.4 software was employed to perform a meta-analysis of the included studies.A total of 30 retrospective clinical controlled studies were included,encompassing 3117 strains,with 1220 HvKP strains and 1897 non-HvKP strains.Meta-analysis revealed that the detection rates of the following virulence genes were significantly higher in the HvKP group compared to the non-HvKP group(P<0.05):rmpA(OR=65.38,95%CI(33.92,126.01),P<0.001),mrkD(OR=1.73,95%CI(1.06,2.84),P=0.03),aerobactin(OR=148.63,95%CI(43.41,508.90),P<0.001),rmpA2(OR=40.35,95%CI(14.79,110.06),P<0.001),magA(OR=5.32,95%CI(2.98,9.47),P<0.001),iroNB(OR=21.43,95%CI(12.77,35.96),P<0.001),alls(OR=3.85,95%CI(2.88,5.16),P<0.001),kfu(OR=7.05,95%CI(3.74,13.30),P<0.001),entB(OR=5.64,95%CI(1.35,23.53),P=0.02),ybts(OR=2.83,95%CI(1.34,5.97),P=0.006),iucA(OR=51.49,95%CI(3.78,701.27),P=0.003),iutA(OR=48.56,95%CI(14.41,163.65),P<0.001),wcaG(OR=8.95,95%CI(5.98,13.39),P<0.001),and peg-344(OR=37.44,95%CI(0.94,1498.69),P=0.05).In contrast,the detection rates of wabG,fimH,uge,and ureA virulence genes did not show statistically significant differences between the two groups(P>0.05).The detection rates of rmpA,mrkD,aerobactin,rmpA2,magA,iroNB,alls,kfu,entB,ybts,iucA,iutA,wcaG,and peg-344 virulence genes are significantly higher in HvKP strains compared to non-HvKP strains.However,the detection rates of wabG,fimH,uge,and ureA virulence genes have almost equivalent effect between the two groups.
