摘要：目的探究岷县地区2型糖尿病(T2DM)患者中医证型与血脂水平的相关性。方法采用多阶段整群随机抽样方法,对岷县各乡镇居民20~75岁进行横断面调查,并设其中660例T2DM患者为观察组,150名岷县中医院健康体检者为对照组。分析T2DM患者治疗情况、中医证型的分布情况,并比较观察组各中医证型患者与对照组的血脂指标水平。结果岷县DM患者生活综合干预治疗率(63.03%)高于单纯药物治疗率(36.97%)。湿热内蕴及胃热炽盛证在T2DM患者中医证型中占据主要群体,分别为总患病人数的29.39%和28.33%。与对照组比较,肺热津伤、胃热炽盛、肾阴亏虚、气阴两虚、阴阳两虚证患者TC、TG、LDL-C水平均明显升高,HDL-C水平显著降低(P<0.05);而与对照组比较,湿热内蕴证患者的TC、TG、LDL-C水平均显著降低,HDL-C水平显著升高(P<0.05)。结论岷县地区2型糖尿病患者胃热炽盛以及湿热内蕴证为主要证型,血脂指标与中医证型具有相关性。
Objective To explore the correlation between TCM syndromes of type 2 diabetes mellitus (T2DM) and blood lipid level in Min county. Methods A cross-sectional survey was conducted among 20-75 year-old residents in Min county townships by multi-stage cluster random sampling. Six hundred and sixty patients with T2DM were included in observation group and 150 people with physical examination in Traditional Chinese Medicine Hospital of Min County were included in control group. The treatment and the distribution of TCM syndromes in T2DM patients were analyzed, and the levels of blood lipid indexes between TCM syndromes of the observation group and the control group were compared. Results The rate of life comprehensive intervention treatment for diabetic patients in Min county (63.03%) was higher than that of simple drug treatment (36.97%). Damp-heat and stomach-heat syndrome occupied the main group of TCM syndromes in T2DM patients, which were 29.39% and 28.33% of the total number of patients, respectively. Compared with the control group, the levels of TC, TG and LDL-C in patients with lung-heat, stomach-heat, kidney-Yin deficiency, Qi-Yin deficiency and Yin-Yang deficiency increased significantly, HDL-C levels decreased significantly (P<0.05). Compared with the control group, the levels of TC, TG and LDL-C in patients with damp-heat syndrome decreased significantly, and HDL-C levels increased significantly (P<0.05). Conclusion The main syndrome of T2DM patients in Min county area is stomach-heat and damp-heat syndrome. The blood lipid indexes are correlated with TCM syndromes.
