摘要：混合痔是肛肠科常见疾病之一,是指内痔和相应部位的外痔相互融合并累及齿线,兼有内痔和外痔的临床表现。目前,该病治疗方面以手术治疗为主。但由于术后创口感染、手术对肛管直肠周围组织的损伤、患者体质虚弱、术后换药不妥等因素,导致患者术后出现疼痛、水肿、出血等并发症,这些并发症严重影响了患者术后创面愈合的速度和时间,导致术后创面愈合缓慢。术后创面愈合缓慢是指患者术后创口不能在预期时间内顺利愈合。其临床表现为创面分泌物较多,创面呈苍白或紫黑色,肉芽组织水肿等。中医治疗术后创面愈合缓慢以内治法和外治法为主。中医内治法根据整体观念和外科疾病的发生发展过程进行辨证施治,遵循消、托、补三个总的治疗原则,选用适当的方药促进术后创面愈合。中医外治法同样以整体观念、疾病发展不同阶段进行辨证施治,运用药物、物理方法等直接作用于患病部位而达到治疗目的。中医疗法可以预防和改善混合痔术后并发症,促进术后创面愈合。文章系统查阅了中国知网、万方数据库及维普网中有关中医疗法促进混合痔术后创面愈合的相关文献,以期有助于明确中医疗法的优势和不足,为临床诊疗提供参考。
Mixed hemorrhoids is one of the common diseases in the anorectal department. It refers to the fusion of internal hemorrhoids and corresponding external hemorrhoids, involves the dentine, and has the clinical manifestations of both internal and external hemorrhoids. At present, the main treatment of this disease is surgery. However, due to postoperative wound infection, surgical damage to perianal tissue, patients’ physical weakness, improper dressing change after surgery and other factors, patients suffer from postoperative complications such as pain, edema and bleeding, which seriously affects the speed and time of wound healing after operation, resulting in slow wound healing after operation. Slow postoperative wound healing means that the patient’s postoperative wound cannot heal smoothly within the expected time. Its clinical manifestations include more wound secretion, pale or purple-black wound, edema of granulation tissue, etc.. The TCM treatment of slow wound healing is mainly internal treatment and external treatment. The internal treatment of traditional Chinese medicine is based on the overall concept and the occurrence and development of surgical diseases. Following the three general treatment principles of elimination, support and supplementation, appropriate prescriptions are selected to promote postoperative wound healing. External treatment of traditional Chinese medicine is also based on the overall concept of disease development at different stages and syndrome differentiation and treatment, and drugs and physical methods are applied to directly act on the affected part and achieve the therapeutic purpose. Traditional Chinese medicine can prevent and ameliorate postoperative complications of mixed hemorrhoids and promote wound healing. This article systematically reviews the relevant literature on the promotion of wound healing after mixed hemorrhoid surgery in traditional Chinese medicine in CNKI, Wanfang database and VIP network, in order to clarify the advantages and disadvantages of traditional Chinese medicine and provide a reference for its clinical diagnosis and treatment.
