摘要：目的:观察大黄附子汤加减灌肠对糖尿病肾病患者肾功能的影响。方法:选取符合纳入标准的糖尿病肾病Ⅳ期患者72例,按照随机数字表法分为对照组(n=36)与治疗组(n=36),两组患者均给予饮食、运动疗法,对照组给予门冬胰岛素30注射液早餐前12单位,晚餐前12单位,皮下注射;苯磺酸氨氯地平5mg口服,1次/d;酒石酸美托洛尔12.5mg口服,1次/d,在此基础上治疗组应用大黄附子汤加减灌肠,2次/d。两组患者均周连续治疗5天,疗程4周。于4周后观察两组治疗效果、症状变化及肾功能指标,并统计治疗结果。结果:两组中医证候积分均有所下降,其中治疗组中医证候积分下降明显优于对照组(P<0.05);两组肌酐、血尿素氮、24h蛋白量,在治疗后均有下降,与对照组相比,治疗组肌酐、血尿素氮、24h蛋白量下降更明显(P<0.05)。4周后治疗组总有效率为91.6%,对照组总有效率为75.0%,治疗组有效率明显优于对照组(P<0.05)。结论:大黄附子汤加减灌肠对糖尿病肾病中晚期患者肾功能改善显著,值得推广应用。
Objective:To observe the effect of rhubarb Fuzi Decoction plus Decoction on renal function in patients with diabetic nephropathy.Methods:select into standardⅣstage of diabetic nephropathy patients 72 cases,according to random number table method is divided into the control group(n=36)and treatment group(n=36),two groups of patients were given diet,exercise therapy,the control group on the basis of the given hypoglycemic,The control group was given 12 units of insulin 30 injection before breakfast and 12 units before dinner.Amlodipine besylate 5 mg oral daily;Metoprolol tartrate 12.5 mg orally once a day,the treatment group treats in the foundation in the control group on the application of rhubarb tonga enema reduction of lateral root treatment,twice a day.Patients in both groups were treated for 5 days consecutively,and the course of treatment was 4 weeks.The therapeutic effect,symptom changes and renal function indexes of the two groups were observed after 4 weeks,and the therapeutic effect was counted.Results:After 4 weeks,the total effective rate was 91.6%in the treatment group and 75.0%in the control group(P<0.05);The score of TCM symptoms in the treatment group was significantly lower than that in the control group(P<0.05);The quantification of creatinine,blood urea nitrogen and 24-hour protein in the treatment group was significantly lower than that in the control group(P<0.05).Conclusion:Rhubarb Fuzi Decoction has significant effect on renal function in patients with advanced diabetic nephropathy.
