摘要：目的观察舒肝解郁胶囊在围绝经期抑郁症中的应用效果。方法2020年6月—2023年4月,将98例白银市第一人民医院收治的围绝经期抑郁症患者分为采用氟哌噻吨美利曲辛片治疗的常规组(49例)和联合舒肝解郁胶囊治疗的联合舒肝解郁组(49例),分组方法为随机数字表法,所有患者均治疗8周。统计两组治疗8周后的临床疗效,比较两组治疗前和治疗8周后肝功能、甲状腺功能、多导睡眠图指标、抑郁症状及围绝经期症状严重程度。结果治疗8周后,联合舒肝解郁组总有效率高于常规组(P<0.05)。与治疗前比较,联合舒肝解郁组治疗8周后血清谷丙转氨酶(alanine aminotransferase,ALT)、游离三碘甲状腺原氨酸(free triiodothyronine,FT3)水平升高,且高于常规组(P<0.05)。与治疗前比较,两组治疗8周后快速眼动(rapid eye movement,REM)睡眠时间、睡眠总时间延长,联合舒肝解郁组长于常规组(P<0.05);与治疗前比较,睡眠潜伏期、醒觉时间缩短,联合舒肝解郁组短于常规组(P<0.05);睡眠效率升高,联合舒肝解郁组高于常规组(P<0.05)。与治疗前比较,两组治疗8周后汉密顿抑郁量表(Hamilton depression scale,HAMD)、贝克抑郁问卷(Baker depression questionnaire,BDI)、Kupperman评分降低,联合舒肝解郁组低于常规组(P<0.05)。结论舒肝解郁胶囊有助于调节围绝经期抑郁症患者机体甲状腺功能,改善患者睡眠质量、抑郁症状及围绝经期症状,临床疗效较好,但会引起部分患者ALT水平轻度升高。
Objective To observe the effect of Shugan Jieyu Capsule(舒肝解郁胶囊)on perimenopausal depression.Methods From June 2020 to April 2023,98 patients with perimenopausal depression admitted to the Baiyin First People′s Hospital were divided into a conventional group treated with haloperthixol melitrixen tablet(49 cases)and a combinedgroup treated with Shugan Jieyu Capsule(49 cases).All patients were treated for 8 weeks by random number table method.The clinical efficacy of the two groups after 8 weeks of treatment was analyzed,and the liver function,thyroid function,polysomnography indicators,depressive symptoms and perimenopausal symptoms were compared between the two groups before and after 8 weeks of treatment.Results After 8 weeks of treatment,the total effective rate of the combined group was higher than that of the conventional group(P<0.05).The serum levels of alanine aminotransferase(ALT),free triiodothyronine(FT3)in the combined group after 8 weeks of treatment were higher than those before treatment,and higher than those in the conventional group(P<0.05).After 8 weeks of treatment,the rapid eye movement(REM)sleep time and total sleep time of the two groups were longer than those of the conventional group(P<0.05).Compared with before treatment,the sleep latency and wake time were shortened,and the combined Shugan Jieyu group was shorter than the conventional group(P<0.05).Compared with those before treatment,the sleep efficiency was higher than that of the conventional group(P<0.05).After 8 weeks of treatment,the scores of Hamilton depression scale(HAMD),Baker depression questionnaire(BDI)and Kupperman in both groups were lower than those before treatment,and the scores of the combined group were lower than those of the conventional group(P<0.05).Conclusion Shugan Jieyu Capsule could help to regulate the thyroid function of patients with perimenopausal depression,improve the sleep quality of patients,depression symptoms and perimenopausal symptoms,and the clinical effect was good,but it could cause a slight increase in level of ALT in some patients.
