摘要：“阳化气,阴成形”理论是对阴阳生理功能的高度概括,具有广泛适用性。田雪梅主任认为,痛风石属有形之体,其性属阴,总因阳气失用,化气无力,由湿浊痰瘀等阴邪凝结而成,病机与“阳化气,阴成形”失常存在高度一致性,“阳化气”不足、“阴成形”太过是痛风石形成的病机核心。田雪梅主任还总结出“少火生气,温阳为主”“疏肝解郁,通阳为助”“审因论治,祛邪为辅”“生活调摄,愈后防复”的治疗思路,以标本兼顾。
Theory of“Yang(阳)transforming Qi(气)while Yin(阴)constituting form”constitutes a fundamental summation of Yin-Yang physiological functions with broad clinical implications.Chief physician TIAN Xuemei considers gouty tophus as a tangible substance inherently pertaining to Yin,which consistently results from Yang Qi deficiency and impaired Yang transforming Qi,leading to the congealing of pathogenic factors of Yin nature such as dampness,turbidity,phlegm and stasis.This pathogenesis fundamentally aligns with dysregulation of“Yang transforming Qi while Yin constituting form”.Chief physician TIAN Xuemei identifies insufficient Yang transforming Qiand excessive Yin constituting form as the core pathological mechanism of gouty tophus formation,summarizing a therapeutic approach addressing both root and manifestation,focusing on warming Yang to nourish vital fire for Qi generation;supporting ascending Yang by soothing liver and resolving stagnation;eliminating pathogens through syndrome-differentiated treatment and modifying lifestyle to prevent recurrence post-treatment.
