摘要：近年来肿瘤的发病率在人群中逐渐提高,针灸作为绿色疗法,对恶性肿瘤放化疗后不良反应的改善有着巨大的帮助。本研究对国内近10年针刺减轻恶性肿瘤化疗引起的胃肠道反应、周围神经毒性、骨髓抑制以及改善中医临床症候的作用进行总结和归纳。以'针灸'为主题,'肿瘤''化疗'为关键词,检索中国知网、维普、万方数据库2008-01-01-2018-12-31所收录的相关文献。纳入标准:恶性肿瘤接受化疗后出现不良反应且为临床随机对照试验;排除标准:针刺、艾灸、穴位注射及埋线等>3个疗法并用、个案报道、研究进展、文献综述、学位会议论文以及质量相对较低的文献。共纳入文献35篇。结果表明,针刺治疗在恶性肿瘤放化疗前后起着减毒增效的作用,尤其在胃肠道反应方面,选穴以足三里穴、内关穴为主,针刺手法以补法和平补平泻为主。周围神经毒性方面,更注重电针及中药外洗方的应用。目前针刺辅助治疗确有疗效和应用前景,值得进一步深入研究。在今后的临床研究中,应采用更全面多样的评价系统,对针刺手法选择的研究分析,同时进行多中心、大样本试验。
In recent years,the incidence of cancer has gradually increased in the population.Acupuncture and moxibustion,as a green therapy,has great help to improve the adverse reactions of malignant tumors after radiotherapy and chemotherapy.This study summarizes the role of acupuncture in alleviating gastrointestinal reaction,peripheral neurotoxicity,bone marrow suppression and improving clinical symptoms of traditional Chinese medicine(TCM)caused by chemotherapy for malignant tumors in recent 10 years in China.With the theme of'acupuncture and moxibustion'and the keywords of'cancer'and'chemotherapy',we searched the relevant literature collected by China national knowledge internet,VIP net and Wanfang databases from January 1,2008 to December 31,2018.Inclusion criteria were adverse reactions of malignant tumors after chemotherapy were randomized controlled trials.Exclusion criteria included combination of acupuncture,moxibustion,acupoint injection and catgut embedding>3 therapies,case report,research progress,literature review,dissertation of degree conference and literature of relatively low quality.A total of 35 articles were included.The results showed that acupuncture treatment played a role of reducing toxicity and increasing efficiency before and after radiotherapy and chemotherapy for malignant tumors,especially in gastrointestinal tract reaction.The main points were Zusanli and Neiguan,and the main acupuncture methods were tonifying and Pingbu Pingxie.For peripheral neurotoxicity,more attention should be paid to the application of electro-acupuncture and external Chinese medicine lotion.At present,acupuncture adjuvant therapy has curative effect and application prospects,which deserves further study.In future clinical research,we should adopt a more comprehensive and diverse evaluation system to study and analyze the selection of acupuncture manipulation,and carry out multi-center and large-sample experiments at the same time.
