摘要:中医学将气道黏液高分泌归属于“痰”的范畴。当各种致病因素影响脏腑气化功能,水谷精微不能正常化为气、血、津液,即会凝聚成病理产物“痰”。西医学认为,气道黏液高分泌是各种致病因素引起气道黏膜产生过量黏液的病理生理过程。它会导致纤毛的正常清除功能发生障碍,局部防御功能减弱、致病菌反复感染、气道阻塞,从而加剧易感患者的健康风险,提高发病率和病死率。因此,气道黏液高分泌被视为气道炎性疾病的重要危险因素。然而,临床对其机制尚不明确,阐明其潜在机制并提出安全有效的靶向治疗策略,才能更加高效地指导临床用药。气道黏液高分泌是一个复杂的病理过程,多种炎症细胞、炎症介质及相关的细胞信号通路均可参与其调节。大量研究表明,表皮生长因子受体(Epidermal Growth Factor Receptor,EGFR)的激活是诱导黏液化生和上调黏蛋白(Mucin,MUC)表达的重要信号。EGFR相关的信号传导途径在气道黏液过度分泌的形成和发展过程中起着关键作用,多种刺激均可导致呼吸道上皮细胞中EGFR的表达增加,引起MUC高表达,导致气道黏液高分泌。文章对EGFR相关通路调控气道黏液高分泌以及近年来中医药靶向调控EGFR相关通路防治气道黏液高分泌的研究进行综述,以期为临床治疗提供理论支持。
According to traditional Chinese medicine,airway mucus hypersecretion can be attributed to the category of“phlegm”.When various pathogenic factors affect the Qi(气)transformation of Zang-fu organs(脏腑),the water and grain essence can not transform into blood,body fluid,which will combine and condense to form the pathological product“phlegm”.Western medicine believes that airway mucus hypersecretion is a pathophysiological process in which various pathogenic factors cause excessive mucus production in airway mucosa.It will lead to the dysfunction of normal cilia clearance,weakened local defense function,repeated infection of pathogenic bacteria,and airway obstruction,thereby increasing the health risk of susceptible patients.Therefore,airway mucus hypersecretion is regarded as an important risk factor in the course of airway inflammatory disease.However,the clinical mechanism is not yet clear.Only by elucidating the potential mechanism and proposing safe and effective targeted therapeutic strategies can we guide clinical medication more effectively.The airway mucus hypersecretion is a complex pathological process,and various inflammatory cells,inflammatory mediators,and related cell signaling pathways can be involved in their regulation.A large number of studies have shown that the activation of epidermal growth factor receptor(EGFR)is an important signal to induce mucus metaplasia and upregulate mucin expression.EGFR related signaling pathway plays a key role in the formation and development of airway mucus hypersecretion.A variety of stimuli can lead to increased expression of EGFR in respiratory epithelial cells,causing high expression of mucin,leading to airway mucus hypersecretion.This article reviews the research on EGFR related pathway regulating airway mucus hypersecretion and the prevention and treatment of airway mucus hypersecretion by targeted regulation of EGFR related pathway in traditional Chinese medicine in recent years,in order to provide theoretical support for clinical practice.
