摘要：特发性肺纤维化是一种肺间质受损导致进行性呼吸困难并最终引起死亡的肺疾病,因其具体的病因及发病机制不明确,目前仍不能完全治愈。特发性肺纤维化属于中医学"肺痿""肺痹"等范畴,多由反复外感、感染虫毒、失治误治等外因及年老体虚、素有肺疾、内伤七情等内因所致。从中医学角度阐释特发性肺纤维化的病因病机及治疗,认为该病的发生与痰瘀毒虚等致病因素相关,总体病机为本虚标实,可表现为实证、虚证以及虚实夹杂证,具体证型以"肺虚络瘀"为主,故"扶正通络化瘀"为主要治法。临床主要通过单味药物及提取物、验方、中药注射剂及针灸等方法进行治疗。本文对特发性肺纤维化的病因病机、证型、治法及治疗进行了归纳总结,以期为临床治疗特发性肺纤维化提供参考。
Idiopathic pulmonary fibrosis is a lung disease in which interstitial lung damage leads to progressive dyspnea and eventually causes death.Because its specific etiology and pathogenesis are unclear,it is still not completely cured.Idiopathic pulmonary fibrosis belongs to the categories of"Fei Wei"and"Fei Bi"in traditional Chinese medicine.It is mostly caused by external factors such as repeated external sensation,infection with insect poison,mistreatment,and internal causes such as aging and physical weakness.Explaining the etiology,pathogenesis and treatment of idiopathic pulmonary fibrosis from the perspective of traditional Chinese medicine.It is considerated that the occurrence of the disease is related to pathogenic factors such as phlegm,blood stasis,toxin,and deficiency.The overall pathogenesis is based on deficiency and excess,which can be manifested as excess syndrome,deficiency syndrome and mixed syndrome of deficiency and excess.The specific syndrome type is mainly"lung deficiency,collaterals and blood stasis",so"Fuzheng Tongluo Huayu"is the main treatment.Clinically,it is mainly treated by single medicines and extracts,proven prescriptions,traditional Chinese medicine injections,and acupuncture.This article summarizes the etiology,pathogenesis,syndrome types,treatment of idiopathic pulmonary fibrosis,in order to provide references for clinical treatment of idiopathic pulmonary fibrosis.
