摘要:目的:探讨艾灸联合生肌玉红膏对压疮大鼠皮肤损伤修复的影响及作用机制。方法:36只SPF级SD大鼠均采用定制压疮造模装置通过缺血-再灌注损伤方式建立Ⅱ、Ⅲ期压疮动物模型,判断造模成功后随机分为模型组、艾灸组、生肌玉红膏组、艾灸联合生肌玉红膏组,每组9只,分别于连续干预3、7、14 d计算创面愈合率,观察组织病理情况,采用免疫组化法检测创面组织磷酸化PI3K(p-PI3K)、磷酸化AKT(p-Akt)、磷酸化mTOR(p-mTOR)蛋白表达水平。结果:干预7、14 d,艾灸联合生肌玉红膏组创面愈合率高于生肌玉红膏组、艾灸组、模型组(均P<0.05)。HE染色发现,艾灸联合生肌玉红膏组炎性细胞浸润较少,可见较多新生肉芽组织及大量排列紧密的成纤维细胞。免疫组化结果显示,p-PI3K、p-Akt、p-mTOR蛋白表达水平从干预3 d开始升高,干预7 d达到高点,随后逐渐降低。干预7、14 d,艾灸联合生肌玉红膏组p-PI3K、p-Akt、p-mTOR蛋白表达高于艾灸组、生肌玉红膏组及模型组(均P<0.05)。结论:艾灸联合生肌玉红膏可显著促进压疮大鼠创面修复,改善病理组织形态,其作用机制可能与激活PI3K/Akt/mTOR信号通路,促进血管新生及蛋白质合成,发挥抗凋亡作用有关。
Objective:To investigate the effect and mechanism of moxibustion combined with Shengji Yuhong ointment on skin injury repair in pressure ulcer(PU)rats.Methods:36 SPF SD rats were used to establish stageⅡand stageⅢpressure ulcer animal models by ischemia-reperfusion injury using a customized pressure ulcer modeling device.After successful modeling,they were randomly divided into model group,moxibustion group,Shengji Yuhong ointment group,and moxibustion combined with Shengji Yuhong ointment group,with 9 rats in each group.The wound healing rate was calculated on the 3rd,7th,and 14th days after continuous intervention,and the histopathological conditions were observed.The expression levels of phosphorylated PI3K(p-PI3K),phosphorylated AKT(p-Akt),and phosphorylated mTOR(p-mTOR)proteins in wound tissues were detected by immunohistochemistry.Results:On the 7th and 14th days after treatment,the wound healing rate of the moxibustion combined with Shengji Yuhong ointment group was higher than that of Shengji Yuhong ointment group,moxibustion group and model group(all P<0.05).HE staining showed that there was less inflammatory cell infiltration in the moxibustion combined with Shengji Yuhong ointment group,and more new granulation tissue and a large number of closely arranged fibroblasts were observed.The results of immunohistochemistry showed that the expression levels of p-PI3K,p-Akt and p-mTOR proteins began to increase from the 3rd day,reached the highest on the 7th day,and then gradually decreased.On 7th and 14th days after intervention,the protein expressions of p-PI3K,p-Akt and p-mTOR in moxibustion combined with Shengji Yuhong ointment group were higher than those in moxibustion group,Shengji Yuhong ointment group and model group(all P<0.05).Conclusion:Moxibustion combined with Shengji Yuhong ointment can significantly promote wound repair and improve pathological tissue morphology in rats with pressure sores.The mechanism may be related to the activation of PI3K/Akt/mTOR signaling pathway,promotion of angiogenesis and protein synthesis,and anti-apoptosis.
