摘要:目的观察PDCA管理在微创针刀镜围手术期护理质量持续改进中的应用效果。方法选取行微创针刀镜手术的76例患者为研究对象,2023年11月至2024年3月收治的38例患者为对照组,2024年4月至2024年8月收治的38例患者为观察组。对照组采用常规护理管理,观察组采用PDCA护理管理。比较2组护理管理前后的围手术期健康教育达标率和进步率,以及护理质量(包括护理评估、健康宣教、基础护理及护理文书质量)评分、护士核心能力(包括PDCA管理手法、主观能动性、工作责任心及团队精神)评分、患者对健康教育(包括术前沟通、术后指导、疼痛护理、辨证施膳、功能锻炼和穴位按摩)的掌握情况及对护理工作(包括护理效果、护士责任心、护患沟通、护理方式)的满意度。结果PDCA管理后,围手术期健康教育达标率由管理前的57.48%提高到95.88%,进步率达51.06%。观察组围手术期护理质量评分和护士核心能力评分均高于对照组(P<0.001或P<0.01)。观察组患者对健康教育的掌握情况优于对照组,对护理工作的满意度高于对照组(P<0.05或P<0.01或P<0.001)。结论在微创针刀镜手术患者围手术期应用PDCA管理,有助于提升围手术期护理质量,可提高围手术期健康教育的达标率、护士的核心能力及患者满意度。
Objective To evaluate the application efficacy of PDCA(Plan-Do-Check-Act)cycle management in facilitating continuous quality improvement of perioperative nursing care for patients undergoing minimally invasive needle-knife arthroscopy.Methods A cohort of 76 patients scheduled for minimally invasive needle-knife arthroscopy was enrolled as the research subjects.38 patients admitted from November 1,2023 to March 31,2024 were selected as the control group,and 38 patients admitted from April 1,2024 to August 31,2024 were selected as the observation group.The control group received conventional nursing,while the observation group underwent PDCA management.The perioperative health education compliance rate and improvement rate were compared between the 2 groups before and after nursing management interventions.Analysis was conducted in terms of nursing quality scores(comprising nursing assessment,health education,basic nursing care,and nursing documentation quality),nurse core competency scores(evaluating PDCA management techniques,proactive initiative,work accountability,and team collaboration),patient mastery of health education protocols(encompassing preoperative communication,postoperative guidance,pain management,dietary therapy based on syndrome differentiation,functional exercises,and acupoint massage),and patient satisfaction with nursing care(including assessments of nursing efficacy,nurse accountability,nurse-patient communication,and nursing approaches).Results After PDCA management,there was a significant elevation in perioperative health education compliance rate,rising from 57.48%pre-intervention to 95.88%post-intervention,with an improvement rate of 51.06%.Both perioperative nursing quality scores and nurse core competency scores were higher in the observation group compared to the control group(P<0.001 or P<0.01).Patient mastery of health education protocols and satisfaction with nursing care in the observation group were higher than those in the control group(P<0.05 or P<0.01 or P<0.001).Conclusion The PDCA cycle management during the perioperative period of patients undergoing minimally invasive needle-knife arthroscopy may help enhance the quality of perioperative nursing,improve health education compliance rates,augment nurses’core competencies,and elevate patient satisfaction levels.
