摘要：【目的】探讨中医院脑梗死患者住院费用的构成及其影响因素,为卫生行政部门和医疗机构控制医疗费用过快增长提供数据支持与政策建议。【方法】运用Epidata 3.1进行数据双录入,运用SPSS 22.0统计软件,对选取的2013~2017年2138例脑梗死出院患者住院费用进行描述性统计,对其影响因素从社会特征、临床特征、医院管理因素等方面进行单因素分析和多因素分析。【结果】(1)在2013~2017年度费用总结中,主要费用包括药费、治疗费、检查费、检验费等,占总费用的比例分别为35.28%、34.05%、12.91%、10.46%。(2)经单因素分析,脑梗死患者住院费用的主要影响因素有年龄、入院病情、住院天数、并发症个数和是否手术等,差异有统计学意义(P<0.05或P<0.01)。(3)经多因素分析,住院费用的主要影响因素有住院天数、入院病情(急)、年龄(≥85岁)、医保类型(市医保)、治疗效果(好转)、伴随病个数和是否手术等,差异有统计学意义(P<0.05或P<0.01)。【结论】在保证患者疗效的前提下,脑梗死患者的住院费用控制重点应放在缩短住院天数、减少不合理用药和治疗、制定合理的单病种管理措施等方面,从而控制患者住院费用的不合理增长,减轻患者的经济负担。
Objective To investigate the composition of hospitalization expenses and the influencing factors in patients with cerebral infarction in a Chinese medicine hospital, so as to provide data support and policy recommendations for health administrative departments and medical institutions to control the excessive growth of medical expenses. Methods Epidata 3.1 was used for data double entry. SPSS 22.0 software was used for the descriptive analysis of the hospitalization expenses of 2 138 discharged patients with cerebral infarction from the year of 2013 to 2017. Single factor analysis and multivariate analysis were performed for the investigation of the influencing factors of social characteristics,clinical features,and hospital management factors and so on. Results(1)In the 2013-2017 annual fee summary,the main expenses included the drug cost,treatment fee,inspection fee,and laboratory examination cost,which accounted for 35.28%,34.05%,12.91% and 10.46%,respectively.(2)The single factor analysis showed that the main influencing factors of hospitalization expenses were age,admission illness conditions,hospitalization days, number of accompanying diseases,surgery,etc.(P<0.05 or P<0.01).(3)The multivariate analysis showed that the main influencing factors of hospitalization expenses were hospitalization days, admission illness conditions(urgent), age(≥ 85 years old), type of medical insurance(city medical insurance), treatment effect(improvement), number of accompanying diseases,surgery,etc.(P<0.05 or P<0.01). Conclusion On the premise of ensuring the clinical efficacy of patients,the key to controlling the hospitalization expenses for patients with cerebral infarction is to shorten the hospitalization days,reduce irrational use of drugs and treatment,and develop reasonable management measures for single disease,so as to control the unreasonable growth of hospitalization expenses and to reduce the patients’ economic burden.
