摘要：小儿遗尿症病位在膀胱,病机为三焦气化失司,膀胱约束不利,其病因包括肾气不固、下元虚寒、膀胱失约等,但与情志因素密切相关。若喜、怒、悲、思、恐五志过极则易影响五脏气机而诱发或加重遗尿,故临床当在重视肾与膀胱的基础上,加强患儿情志的调节。治疗方面可以运用情志相胜论,选取相对应的情志以偏纠偏,同时针对患儿肺脾肾常不足,应注重其体质调节;心肝有余的患儿,若心窍不开当开窍醒神,肝郁化火当清肝解郁。
Enuresis in children is located in the bladder,with the pathogenesis being a disorder of the qi transformation in the three burners and improper restraint of the bladder.Its etiology includes instability of kidney qi,deficiency-cold in the lower jiao,and bladder dysfunction,but it is closely related to emotional factors.When the five emotions-joy,anger,sorrow,contemplation,and fear-are experienced to excess,they can easily affect the qi mechanism of the five zang organs and induce or exacerbate enuresis.Therefore,in clinical practice,in addition to focusing on the kidney and bladder,it is essential to strengthen the regulation of children's emotions.In terms of treatment,the theory of emotional counteraction can be applied by selecting corresponding emotions to correct imbalances.Meanwhile,considering that children often have insufficient lung,spleen,and kidney qi,attention should be paid to regulating their constitution.For children with excessive heart and liver qi,if the heart orifice is obstructed,methods to open the orifice and awaken the spirit should be used,and if liver stagnation transforms into fire,methods to clear the liver and relieve depression should be applied.
