摘要:目的评估碘克沙醇与碘海醇在增强CT检查(CECT)中导致造影剂相关性肾病(CIN)的发生率差异,并分析合并用药对CIN发生的影响。方法回顾性分析2021年1月—2024年6月在甘肃中医药大学附属医院接受CECT检查的560例患者资料,分为碘克沙醇组(n=280)和碘海醇组(n=280),对比分析两组CIN发生情况。结果两组CIN总体发生率分别为25.71%和26.07%,差异无统计学意义(P>0.05)。服用二甲双胍患者CIN发生率显著高于未服用者(P<0.05)。结论两种造影剂CIN发生风险相当,但合并用药(尤其是二甲双胍)可能增加CIN发生率,临床选择造影剂时应考虑患者用药史及基础状况。
Objective To evaluate the difference in CIN(Contrast-Induced Nephropathy)incidence between iodixanol and iohexol in CECT(Contrast-Enhanced CT)examinations,and analyze the impact of concurrent medications on CIN occurrence.Methods A retrospective analysis was conducted on 560 patients who underwent CECT examinations at the Affiliated Hospital of Gansu University of Chinese Medicine from January 2021 to June 2024.Patients were divided in to iodixanol group(n=280)and iohexol group(n=280),and the incidence of CIN was comparatively analyzed between the two groups.Results The overall CIN incidence rates were 25.7% and 26.1% in the two groups respectively,with no statistically significant difference(P>0.05).Patients taking metformin showed a significantly higher CIN incidence rate(approximately 31.5%)compared to those not taking metformin(approximately 19.0%)(P<0.05).Conclusion Both contrast agents carry similar risks of CIN,however,concurrent medications(especially metformin)may increase the incidence of CIN.Clinical selection of contrast agents should consider the patient's medication history and baseline conditions.
