摘要:目的:观察在拔伸按压过程中对腰椎结构生物力学变化的影响,探讨拔伸按压手法治疗腰椎间盘突出症的作用机理,明确该手法治疗腰椎间盘突出症的合理性和有效性。方法:实验材料来源于急性脑死亡的健康青壮年尸体的6例腰椎标本,参考临床拔伸按压手法在体研究的力学参数及本手法在临床操作的基本动作为依据设计加载方案,测量拔伸按压过程中L3~4、L4~5、L5S1节段髓核内压力的变化、L3~4、L4~5、L5S1节段椎间距前、后缘高度的的变化、双侧L4~5椎间孔最大宽度、最小宽度及高度的变化和双侧L5神经根与椎间盘外后方之间位移变化。结果:在手法过中,三节段髓核内压力明显降低、椎间距增大、椎间孔容积增大、神经根发生一定的位移。结论:拔伸按压手法符合生物力学原理,治疗腰椎间盘突出症是合理的、有效的,值得在临床上推广应用。
To observe the biomechanical changes of lumbar spine structure by the metnoa of tracnon ana pressing technique, and to detect its mechanism and effect. Methods: Six lumbar spines samples derived from healthy young adults corpses of acute brain death. The experimental project was designed by the mechanical parameters and clinical manipulation of traction and pressing technique in vivo. In the process of traction and pressing technique, the changes of those indices such as pressure in the nucleus pulposus (NP) and the ahead and rear height of intervertebral spacing of L3/4, L4/5 and L5/ S1 spine segments, the width and height of both sides of intervertebral foramina of L4/5 spine segment, and the displacement of neural root to posterolateral lumbar were measured. Results ; During the procedure of technique, the pressure inside NP was decreased obviously, intervertebral spacing and intervertebral foraminal dimension were increased, and neural root was motioned. Conclusion : The traction and pressing technique is complying with principle of biomechanics for treating the lum- bar intervertebral disc prolapse, and it is reasonable and effective.
