摘要：子宫内膜异位症(EMs)是常见的妇科病。该病症可以导致高达30%~50%的EMs患者不孕,严重影响女性健康和生活质量。如何在临床治疗过程中,运用有效的治疗方法提高患者的生育能力是备受医学界关注的问题。本文概述了子宫内膜异位症不孕的中、西医治疗方法,中西结合治疗方法及各自的疗效并指出中西医结合治疗该病症的优势。各治疗方法及疗效如下。中医治疗方法有辨证治疗、专方治疗、周期治疗。辨证治疗的方法主要是补肾化瘀法和活血化瘀法。就补肾化瘀法文中介绍了贺希的报道及岳会群的对照试验,就活血化瘀法文中介绍了郝峻峰成功治疗继发性不孕患者的案例及曾静雯的EMs大鼠模型试验。在专方治疗方面,本文中列举了韩冰、夏天和蔡小荪在治疗子宫内膜异位症不孕时所采取的专方及疗效。在周期治疗方面,本文中概述了王子瑜、刘海燕、刘蓝笛各自在经前、经期、经后所采取的不同治疗方法和方药及其疗效。西医治疗方法有期待疗法、药物疗法和手术疗法。期待疗法因自然妊娠率极低,不主张采用。药物治疗并不能提高患者的生育能力,只能延缓复发。手术治疗是采用腹腔镜手术,该治疗方法能有效提升妊娠率,但术后易复发。中西医结合治疗的方法有中、西药结合治疗和手术联合药物治疗。在中、西药结合治疗方面文中介绍了付蒙、郭丽娜、贾素敏、陈丹利用中、西药结合的方法成功治疗子宫内膜异位症不孕的案例及实验;在手术联合药物治疗方面,文中介绍了蒋袁园的腹腔镜手术联合药物治疗对照试验和张会美、叶青、曾诚在腹腔镜手术联合药物治疗方面的观点。
Endometriosis （EMs） is a common gynecological disease. The disease can cause infertility for 30% ~ 50% patients with EMs, which has serious impact on women＇s health and life. How to use effective treatments to improve patients＇ fertility is a problem concerned by medical profession. This article provides an overview about the therapeutic methods and their efficacy on the infertility caused by endometriosis, involving western treatments, TCM treatments and the integrative medicine treatments. The advantages of the integrative medicine in treating the disease are also pointed out in this thesis. The therapeutic methods and efficacy are as follows. There are differential treatment, special treatment and periodic treatments of TCM therapies. Differential treatment refers to the therapy which can remove blood stasis by invigorating kidney or promoting blood circulation. This paper presents He Xi＇s report and Yue Huiqun＇s controlled trials. As for the promoting blood circulation to remove blood stasis therapy, this paper introduces Hao Junfeng＇s clinical cases in treating secondary infertility successfully and Zeng Jingwen＇s EMs rat model test. In terms of special treatment, the paper cites Han Bing＇s, Xia Tian＇s and Cai Xiaosun＇s special treatments in treating infertility caused by endometriosis and their efficacy. As for periodic treatments, the therapy and prescription by Wang Ziyu, Liu Haiyan and Liu Landi in dealing with infertility caused by endometriosis during premenstrual, menstrual period and after menstrual period are outlined in this article. Western treatments include expectant therapy, medicine therapy and surgical therapy. Expectant therapy is not advocated for its extremely low natural pregnancy rates. Medicine therapy can not improve the patient＇s reproductive capacity although it can delay the recurrence. Surgical treatment is the use of laparoscopic surgery; the treatment can improve pregnancy rates, but would relapse after surgery. The integrative medicine treatments include the combination of TCM and Western medicine approach as well as the combination of medicine and surgery approach. As for the integrative medicine approach, this paper cites Fu Meng＇s, Guo Lina＇s, Jia Sumin＇s, Chen Dan＇s clinical cases and experiments in treating infertility caused by endometriosis. In terms of the combination of medicine and surgery approach, Jiang Yuanyuan＇s compared experiment about laparoscopic surgery combined medicine is introduced. Besides, Zhang Huimei＇s, Ye Qing＇s and Zengcheng＇s opinions are also cited to show the good efficacy of the combination of medicine and surgery approach.
