摘要：目的:评价闭合减压刺骨术治疗膝关节退行性变的临床疗效;方法:纳入膝关节退行性变患者70例,采用临床随机对照研究方法,将患者分为闭合骨减压组和对照组各35例。闭合骨减压组行刺骨减压治疗,隔日1次,共治疗4周。对照组接受双氯芬酸二乙胺软膏涂擦患处治疗,每日1次,疗程4周。两组患者分别在治疗前后利用膝关节功能评分(AKSS量表)、膝关节主动屈伸度、髌骨被动活动度、满意度调查等综合评价两种干预措施治疗本病的临床疗效,并比较疗效的优越性。结果:70例患者依从性较高,无脱落病例。两组患者治疗后AKSS评分明显提高、膝关节主动屈伸度、髌骨被动活动度显著改善,在症状缓解方面,闭合减压刺骨术明显优于药物贴敷治疗组(P<0.05)。治愈率方面,骨减压组的临床有效率达到88.6%,药物贴敷治疗组患者治愈率为77.1%,提示闭合减压刺骨术的临床有效率明显优于药物治疗组(P<0.05)。结论:闭合减压刺骨术能够在短期内改善膝关节退行性变患者的疼痛症状,显著提高患者的膝关节活动能力,其临床治愈率高,疗效优于双氯芬酸二乙胺软膏涂擦治疗,同时避免了使用NSAIDs类药物的不良反应及并发症,安全性较高,值得推广和应用。
Objective：To evaluate the clinical effect of bone puncture decompression on knee joint degeneration.Methods：70cases of knee degenerative changes were divided into closed bone decompression group and control group,35 cases in each group.Closed bone decompression group underwent a decompression treatment,1every other day,a total of 4weeks of treatment.The control group received diclofenac two ethylamine ointment treatment,1time daily for 4weeks.Some index were evaluated such as the knee function score（AKSS scale）,active flexion and extension degree of the knee joint,patella passive mobility and satisfaction degree in the two groups of patients before and after treatment.Results：70cases of patients with high dependence,no shedding cases.After treatment,the AKSS score of the two groups was significantly improved,the active flexion and extension of the knee and the active patella were significantly improved.In the relief of symptoms,the closed bone puncture decompression was significantly better than the drug treatment group（P〈0.05）.The clinical cure rate of the bone decompression group reached 88.6%,and the cure rate was 77.1%,which indicated that the clinical cure rate of the closed decompression was better than the drug treatment group（P〈0.05）.Conclusions：Closed bone puncture decompression can rapidly improve the pain symptoms of patients with degenerative knee joint,and significantly improve the patients knee joint activity.At the same time,it avoid the adverse reactions and complications of the use of NSAIDs drugs,and is worthy of promotion and application in the future.
