摘要：目的:通过检测胃癌癌前病变患者三种不同中医证型(脾胃虚弱证、阴虚内热证、胃络瘀阻证)胃黏膜组织中抑癌基因PTEN的表达,探讨中医证型与PTEN表达之间的相关性。方法:将实验患者分为对照组和实验组,对照组为26位健康体检者,取其正常胃组织标本,实验组为不同中医证型患者(脾胃虚弱证、阴虚内热证、胃络瘀阻证),故共有4组(健康对照组、脾胃虚弱证组、阴虚内热证组、胃络瘀阻证组);每位患者取新鲜胃黏膜6块,其中包括胃体、胃角和胃窦各2块。新鲜组织经甲醛固定、石蜡包埋后,做连续切片,组织厚度为4μm;采用S-P染色法进行染色,DAB显色,苏木精复染,封片实验用已知阳性片作阳性对照,用PBS代替一抗作阴性对照。由病理医师观察,阳性反应判定标准:PTEN表达为胞浆中出现棕黄色颗粒为阳性细胞。阳性细胞占10%以上为阳性表达,<10%为阴性。结果:PTEN阳性表达在脾胃虚弱证中有85.71%,阴虚内热证中有41.30%,胃络瘀阻证中有23.08%,健康对照组中有96.16%。PTEN阳性在I型肠化生表达为32.07%,Ⅱ型肠化生表达为28.30%,Ⅲ型肠化生表达为24.53%,异型增生表达为15.09%。结论:胃黏膜组织抑癌基因PTEN的表达与胃癌癌前病变的病理类型有关,而且在胃癌癌前病变不同中医证型中表达的差异有统计学意义,可为胃癌癌前病变的中医证型客观化提供依据。
Objective:PTEN gene expression in gastric mucosa in different TCM syndromes of PLGC was tested,to investigate relationship between the different TCM syndromes and PTEN gene.Methods:There are four groups,namely as the Piwei Xuruo group and Yinxu Neire group,Weiluo Yuzu group and healthy control group which is consist of26healthy persons.6fresh gastric mucosa tissues were selected(each two tissues of sinuses ventriculi,gastric angle and corpus ventriculi).Immunohisto chemical staining and PTEN gene expression were observed.Results:The positive rates of PTEN gene expression are85.71%in the Piwei Xuruo group,41.30%in the Yinxu Neire group,23.08%in the Weiluo Yuzu group and96.16%in the healthy control group.The positive rates of PTEN gene expression are32.07%,28.30%,24.53%,15.09%in intestinal metaplasiaⅠ,Ⅱ,Ⅲand dysplasia.Conclusion:The PTEN gene expression in gastric mucosa tissues is pertinent to the pathological types of precancerous lesions of gastric cancer,and has statistical significance in different TCM syndromes of PLGC,which will provide basis for the objective research of different TCM syndromes of PLGC.
