摘要:目的在“病证结合理论”指导下,通过“内湿+外湿+病理因素”的方法建立并评价糖尿病足溃疡(Diabetic foot ulcer,DFU)湿热毒盛证动物模型。方法通过高糖高脂饲料、20%蔗糖水、高温高湿环境饲养,经链脲佐菌素(Streptozotocin,STZ)诱导建立糖尿病大鼠模型的基础上,在足底手术致创并注射金葡萄球菌构建糖尿病足溃疡湿毒证大鼠模型。造模后通过观察大鼠的一般情况、旷场行为学、外观体征评分、舌象变化、一般湿毒证评分及足底创面的改变等宏观角度评价模型。ELISA检测各组大鼠血清中的白细胞介素-6(Interleukin-6,IL-6)、肿瘤坏死因子-α(Tumour necrosis factor-alpha,TNF-α)、C反应蛋白(C-reactive protein,CRP)等炎症因子的含量。取足底溃疡创面和肌腱进行HE染色。结果模型组大鼠与湿热毒盛证候相关的指标如体重等显著降低(P<0.01),血糖、饮水量、进食量及血液中IL-6、TNF-α、CRP等炎症因子的含量显著升高(均P<0.01),一般外观体征评分可达11.5分,舌象表现为深红色,溃疡创面和肌腱组织炎症浸润明显。生肌玉红膏纱条干预后,显著降低了湿热毒盛证候宏观指标(P<0.01),外观体征评分降至7分,血液中炎症因子及溃疡面及肌腱组织炎症浸润程度明显减轻。结论该造模方法进一步验证了湿热毒盛证动物模型,基于体征的半定量指标可以较为客观的评价模型。
Objective To establish and evaluate an animal model of the“syndrome of exuberant dampness-heat toxin”in diabetic foot ulcer(DFU)using a method combining internal dampness,external dampness,and pathogenic factors,guided by the“disease-syndrome combination theory”.Methods Based on a diabetic rat model involving a high-sugar/high-fat diet,20%sucrose water and a high-temperature/high-humidity environment,the model was further constructed by inducing streptozotocin(STZ)and creating a plantar wound via surgery,followed by injecting Staphylococcus aureus to induce the dampness-toxin syndrome.After modelling,evaluation of the model was performed macroscopically by observing the rats’general condition,open-field behavioral test results,appearance and signs score,changes in tongue manifestation,general dampness-toxin syndrome score and alterations to the plantar wound.Serum levels of the inflammatory cytokines interleukin-6(IL-6),tumour necrosis factor-alpha(TNF-α)and C-reactive protein(CRP)were detected using ELISA.HE staining was performed on the plantar ulcer wound and tendon tissues.Results Compared to the control group,the model group showed decreased weight and other indicators associated with the exuberant dampness-heat toxin syndrome(P<0.01).Meanwhile,blood glucose,water intake,food intake,and serum levels of IL-6,TNF-α,and CRP were all significantly elevated(P<0.01).The appearance and signs score reached 11.5 points.Tongue manifestations showed a deep-red color.Significant inflammatory infiltration was evident in the ulcer wound and tendon tissues.Following intervention with Shengji Yuhong Gauze Strips(Shengji Yuhong Ointment Gauze),macroscopic indicators of the syndrome significantly decreased(P<0.01),the appearance and signs score decreased to 7 points,and serum inflammatory cytokine levels were markedly lowered.The degree of inflammatory infiltration in the ulcer and tendon tissues was also significantly alleviated.Conclusion This modeling method further verified the animal model of damp-heat and excessive toxicity syndrome.The semi-quantitative indicators based on physical signs can objectively evaluate the model to a considerable extent.
