摘要:骨缺损常见于创伤、肿瘤切除和感染性骨病,尽管通过传统治疗可有所改善,但存在骨源有限、并发症多等问题。骨缺损修复不仅是成骨与破骨的平衡过程,还涉及炎症反应与骨重建的相互作用。Nod样受体蛋白3(NLRP3)炎症小体介导的细胞焦亡在这一过程中起关键作用,适度炎症和细胞焦亡有助于修复启动,而过度的慢性炎症和细胞焦亡则可能延迟修复。中药单体凭借其多靶点调控特性,在NLRP3炎症小体调控、异常细胞焦亡与炎症反应抑制、骨修复微环境重塑方面展现出显著潜力。通过概述NLRP3炎症小体与细胞焦亡的基本机制,NLRP3炎症小体介导的细胞焦亡在骨缺损修复中的“双刃剑”作用,中药单体调控NLRP3炎症小体介导的细胞焦亡的相关研究进展,为骨缺损的临床治疗提供理论依据和研究方向。
Bone defects frequently arise from trauma,tumor resection,and infectious bone diseases.Despite advancements in conventional treatments,challenges such as limited availability of bone grafts and various complications continue to persist.The repair of bone defects is not only a balanced process involving osteogenesis and osteoclastogenesis but also encompasses the intricate interplay between inflammatory responses and bone remodeling.The NLRP3 inflammasome-mediated pyroptosis plays a pivotal role in this context,moderate inflammation and pyroptosis can facilitate the initiation of repair processes,whereas excessive chronic inflammation and pyroptosis may hinder healing.Monomeric compounds derived from traditional Chinese medicine exhibit significant potential due to their multi-target regulatory properties,which can modulate the NLRP3 inflammasome,suppress abnormal pyroptosis and inflammatory responses,thereby remodeling the microenvironment conducive to bone repair.By elucidating the fundamental mechanisms underlying the NLRP3 inflammasome and pyroptosis,as well as highlighting the dual-edged sword nature of NLRP3 inflammasome-mediated pyroptosis in bone defect repair alongside recent research advancements regarding its regulation by monomer of traditional Chinese medicine,this review aims to provide a theoretical foundation and direction for future clinical approaches to treating bone defects.
