摘要:磷脂酰肌醇3-激酶(PI3K)/蛋白激酶B(Akt)信号通路作为调节细胞生长、存活和代谢等生物过程的关键通路,在勃起功能障碍的形成与发展中发挥了重要作用,其异常激活或失活可能引起阴茎海绵体平滑肌细胞过度增殖及血管和神经的损伤。现代医学对勃起功能障碍的治疗仍未有治愈的手段,中医药以其多成分、多通路、多靶点的特点,在勃起功能障碍的预防和治疗方面展现出日益突出的优势。近年来,多种中药单体、提取物及中药复方等被证实能够通过调节海绵体平滑肌功能、减轻氧化应激和炎症反应、抑制细胞凋亡、调节神经内分泌功能等机制,有效预防和治疗勃起功能障碍。文章基于PI3K/Akt通路,综述了中医药对勃起功能障碍的改善作用,探讨其具体作用机制,以期为中医药治疗勃起功能障碍提供新思路。
The phosphatidylinositol 3-kinase(PI3K)/protein kinase B(Akt)signaling pathway is a crucial regulator of biological processes,including cell growth,survival,and metabolism,and is significantly involved in the pathogenesis of erectile dysfunction(ED).Dysregulation of this pathway may lead to excessive proliferation of penile cavernosal smooth muscle cells and impairment of vascular and neural integrity.Modern medicine offers no definitive cure for ED.Traditional Chinese medicine(TCM),characterized by multicomponent,multi-pathway,and multi-target actions,has demonstrated growing potential in the prevention and treatment of ED.In recent years,various Chinese medicinal monomers,extracts,and compound formulas have been shown to alleviate ED through mechanisms including regulation of cavernosal smooth muscle function,reduction of oxidative stress and inflammatory responses,inhibition of cell apoptosis,and modulation of neuroendocrine function.Focusing on the PI3K/Akt pathway,this review summarizes the therapeutic effects of TCM on ED and explores the underlying mechanisms,aiming to provide new perspectives for ED management with TCM.
