摘要：目的:探讨补肾益骨方辨证加减联合碳酸钙D3片对围绝经期骨质疏松症患者的临床作用。方法:将120例围绝经期骨质疏松症患者按随机数字表法分为试验组与对照组各60例,对照组碳酸钙D3片治疗,试验组另加补肾益骨方辨证加减治疗,比较中医证候积分、临床疗效、骨密度(BMD)与血清抗酒石酸酸性磷酸酶5b(TRACP 5b)、骨特异性碱性磷酸酶(BAP)、Ⅰ型胶原C端肽(CTX-Ⅰ)、骨钙素(BGP)、促卵泡生成激素(FSH)、雌二醇(E2)水平和骨折发生情况。结果:试验组治疗后中医证候积分及血清TRACP 5b、CTX-Ⅰ、FSH水平均明显低于治疗前及对照组,试验组治疗后L1-L4腰椎、左侧股骨颈的BMD及血清BAP、BGP、E2水平均明显高于治疗前及对照组,试验组总有效率高于对照组,而治疗后1年内骨折发生率低于对照组差异显著。结论:补肾益骨方辨证加减联合碳酸钙D3片可明显改善围绝经期骨质疏松症患者临床症状、骨代谢及雌激素水平,提高骨密度疗效显著,并可预防骨折。
Objective: To investigate the clinical effect of Bushenyigu recipe syndrome differentiation addition and subtraction combined with Calcium Carbonate and Vitamin D3 Tablets on patients with perimenopausal osteoporosis. Methods: 120 patients of perimenopausal osteoporosis were randomly divided into experiment group and control group, with 60 cases in each group. The control group was given Calcium Carbonate and Vitamin D3 Tablets, while the experiment group was given the treatment of Bushenyigu recipe syndrome differentiation addition and subtraction on the basis of the control group. The TCM Syndrome integration, clinical efficacy, bone mineral densities(BMD), and the serum tartrate resistant acid phosphatase 5b(TRACP 5b), bone specific alkaline phosphatase(BAP), type I collagen C terminal peptide(CTX-I), bone gamma-carboxyglutamic-acid-containing proteins(BGP), follicle stimulating hormone(FSH) and estradiol(E2) and the incidences of fracture were compared. Results: The TCM syndromes scores and the levels of serum TRACP 5b, CTX-I and FSH of the experiment group after treatment were significantly lower than those before treatment and of the control group. The BMD of L1-L4 lumbar spine and left femoral neck and the levels of serum BAP, BGP and E2 of the experiment group after treatment were significantly higher than those before treatment and of the control group. The total effective rate of the experiment group was higher than that of the control group, and the incidence rate of fracture within one year after treatment of the experiment group was lower than that of the control group, and the differences were significant. Conclusion: Bushenyigu recipe syndrome differentiation addition and subtraction combined with Calcium Carbonate and Vitamin D3 Tablets can significantly improve the clinical symptoms, bone metabolism indexes and estrogen level of patients with perimenopausal osteoporosis, improve bone mineral densities with significant effect, which can also prevent fracture.
