摘要:文章分析整理了近年针刺干预弱视引起视功能重塑调节机制的研究进展,围绕视皮层视神经元形态结构可塑性改变、视觉信号神经编码特征、突触超微结构重塑、神经递质及其受体水平变化、特异性针刺响应基因表达以及单个靶分子、蛋白的生物学效应方面展开评述。现阶段研究成果为针刺治疗弱视特异性穴位选择和疗效判定方面提供了新的思路和方法。同时,也对今后针灸防治弱视的临床推广应用和效应机制的深入研究产生了极大的促进作用。但是,仅从单个基因、蛋白、小分子的静态变化特征中难以全面反映针灸效应的整体性变化特点。今后的研究应重视功能组学研究手段,借助多学科、多方法的综合研究,重点关注特异性针刺响应指标的级联反应通路和集群效应调节特征,这样才能系统全面地揭示针刺对视功能重塑的具体作用机制和效应规律。
This paper analyzes the recent research progress on the regulatory mechanism of acupuncture intervention on visual function remodeling caused by amblyopia. The research topics mainly focus on the changes of morphological and structural plasticity of visual neurons in visual cortex, neural coding characteristics of visual signals, synaptic ultrastructure remodeling, changes of neurotransmitters and their receptors, expression of specific acupuncture response genes, and biological effects of single target molecule and protein. The current research results provide a new idea and method for the selection of specific acupoints and the determination of curative effect in the treatment of amblyopia. At the same time, it also has a great role in promoting the clinical application of acupuncture in the prevention and treatment of amblyopia and the in-depth study of the effect mechanism. However, the acupuncture effect has the characteristics of integrity, and it is difficult to fully reflect the overall change characteristics of acupuncture effect only from the static change characteristics of single gene, protein and small molecule. Therefore, in the future, we should pay more attention to the research methods of functional omics, with the help of multidisciplinary and multi method comprehensive research, focusing on the cascade reaction pathway and cluster effect regulation characteristics of specific acupuncture response indicators. Only in this way can we systematically and comprehensively reveal the specific mechanism and effect law of acupuncture on visual function remodeling.
