摘要:目前,慢性腰痛成为我国主要的公共卫生问题之一。椎间盘退变(intervertebral dise degeneration,IDD)是因细胞外基质(extracellular matrix,EcM)合成减少、椎间盘细胞逐渐衰老和凋亡而牵连引发腰痛的一个主要病理过程,发病普遍,长期存在可导致腰椎管狭窄、腰椎间盘突出及椎体滑脱等。IDD的治疗主要根据腰痛性质进行治疗性干预,这虽然对缓解椎间盘源性腰痛可能有益,但不能从根本上治愈IDD。因此,如何彻底地临床靶向治愈IDD成为迫切解决的科学问题。间质干细胞(mesenchymal stem cells,MSCs)是多能干细胞,具有自我更新能力,能分化成不同的细胞类型,包括软骨细胞谱系,促进ECM形成,免疫调节椎间盘组织炎症反应,分泌相关的生长因子,可间接促进椎间盘组织修复,为椎间盘再生提供可能。本文通过查阅近几年相关MSCs移植在椎间盘再生中应用的文献并进行综合分析,从不同角度阐述IDD的分子机制、病理、治疗现状以及MSCs治疗IDD中存在的问题和前景,旨在突破退变椎间盘不能再生的禁锢,解开传统治疗观念的枷锁,将MSCs治愈IDD的理念引至科学前沿。
With the improvement of people's living standards, chronic low back pain is one of the leading public health problems in China. Degeneration of the intervertebral disc (IDD) is a major pathological process implicated in low back pain, which is characterized by cellular apoptosis and senescence with reduced synthesis of extracellular matrix (ECM). It is ubiquitous and will gradually cause lumbar spinal canal stenosis, disc herniation, and spondylolisthesis, which have led to medical, economi- cal and social problems. At present, the treatment for IDD is mainly to provide therapeutic intervention based on the degree of lum- bago. The treatment is benefit to relieve the lumbago caused by intervertebral disc, but it cannot cure IDD completely. Therefore, how to cure the disease thoroughly in the clinic-targeted way has become an urgent scientific issue that needs to be solved. Mesen-chymal stem cells (MSCs) are pluripotent stem cells, which have ability of self-renewing and differentiating into different cell types, including cartilage cell lineage, promoting the formation of ECM, immuno-modulating the inflammatory response of interver- tebral disc and indirectly enhancing tissue regeneration by the secretion of growth factors. This initiates a new epoch for the regen- eration of intervertebral disc. I have referred, reviewed and analyzed the recent researches related on the application of MSCs transplantation in the regeneration of intervertebral disc domestically and abroad. Therefore, my thesis is going to state molecular mechanism, pathology and treatment status of IDD from different views, as well as the problems and prospects of application of MSCs in treatment of IDD. The aim of my thesis is to break the shackles that degenerated intervertebral disc can' t regenerate, to unlock the imprisonment of the traditional treatment concept, and to lead the idea of applying MSCs for curing IDD to scientific front.
